Job Opportunity: Home Health Physical Therapist (PT)
Location: Walla Walla, Washington
Specialty: Therapy/Rehabilitation
Employment Type: Travel/Contract
Contract Length: 13 weeks
Job Details
- Start Date: ASAP (Compliance process takes about 3 weeks)
- Shift: 8-hour day shifts
- Patient Ratios: 1:1 (may visit up to 6 patients per day)
- Typical Caseload: 4-5 patients per day
- Productivity Standard: 27-30 productivity points based on territory
- Weekend Requirement: None
- On-Call Requirement: None
- Locals Accepted: Yes

Experience & Licensing Requirements
- Minimum 1 year of recent skilled Home Health PT experience within the last 3 years (inpatient experience not accepted)
- Must have a Washington state license or a compact license
- Expert knowledge of OASIS documentation required
- BLS certification required
Additional Information
- Own transportation is required
- Client grouping minimizes drive and windshield time, but occasional travel radius can be up to 100 miles
- Timely communication and documentation are mandatory
- Documentation tools will be provided and must be returned
Compliance & Background Checks
- Background checks must be current within 90 days of start and updated annually, including:
- SSN Trace
- County searches for last 7 years of residence/work/school
- FACIS Level 1M
- Nationwide Sex Offender Search
- Motor Vehicle Records (MVR)
- Education and Work History Verification (work history verification may be waived if background check is grandfathered)
- EPLS/GSA/SAM and HHS/OIG checks required within 90 days of start
- Fingerprinting required for candidates who have not lived in Washington for the past 3 years (completed on-site)
- State-specific criminal history and employability status checks as applicable
Certifications & Verifications
- BLS (Basic Life Support) certification required (AHA, ARC, Military accepted)
- State license verification within 90 days of start and upon expiration
- NURSYS verification for applicable professions (RN, LPN/LVN, AP)
- Competency exams as applicable (medication/pharmacology and specialty competency)
- Signed Orientation Safety Manual Acknowledgement (updated annually)
Health & Immunization Requirements
- Physical exam within the last 12 months (not required annually)
- Fit test for N95 mask (no specific model required)
- Negative TB test (PPD, Q Gold, or TSpot) within 1 year or chest X-ray within 5 years if positive PPD
- Annual TB questionnaire required for all candidates
- Immunizations or positive titers required for:
- Rubeola (Measles)
- Mumps
- Rubella
- Hepatitis B
- Varicella
- 9-panel drug screen within 12 months (instant acceptable)
- Seasonal flu vaccination or facility-specific declination form required during flu season
- COVID-19 vaccination or medical/religious exemption required, including booster or declination form
Documentation & Other Requirements
- Proof of current auto insurance required for candidates with driving responsibilities
- Completion of facility-specific Acceptable Use Policy and Agency Background Attestation prior to start
- Two professional references required within 2 years of start
- Full 7 years of work history documented with no gaps in employment
- Annual skills checklist required
