Occupational Therapist - Home Health
Location: Newton, Massachusetts
Employment Type: Long-Term Assignment (LTA)
Contract Length: 13 weeks
Job Summary
- Provide occupational therapy services to patients in their homes with cognitive and/or physical impairments due to injury, disease, disability, or other causes.
- Patient age range: birth to over 100 years.
- Collaborate with patients, families, physicians, and allied health professionals to assess needs and coordinate care.
- Manage technical, environmental, and financial resources related to patient care.
- Travel required within the geographic area.
Qualifications
- Minimum 1 year certified home care experience.
- Occupational Therapist licensure.
- At least 2 years of experience in acute care, rehab, or homecare as an Occupational Therapist.
- Registered with the Massachusetts Board of Registration.
- Proficient with various computer software; familiarity with handheld patient care devices preferred.
- Strong customer service skills and ability to work with diverse populations.
- Excellent verbal, written, and electronic communication skills.
- Ability to prioritize tasks and adapt to changing situations.
Required Credentials & Health Requirements
- License/Certificate Verification: Primary source verification of professional license/certification required before submission.
- Resume: Include past 7 years of work experience with city and state details.
- References: Two recent references required, at least one supervisor or above in the relevant skillset.
- Skills Checklist: Must be completed and submitted.
- Candidate Submission Coversheet: Required for submission.
Onboarding Requirements (must be completed prior to start)
- Photo ID: Recent color photo (no selfies, plain background).
- Motor Vehicle Record (MVR) Check: Required for all home care positions.
- Fit Test: N95 respirator fit test within 365 days, updated annually.
- Immunizations:
- MMR (measles, mumps, rubella) vaccination or proof of immunity.
- Influenza vaccine during flu season (Sept 15 - April 30), no exemptions.
- TB test (skin or blood test) within 3 months of start date, updated if >6 month break.
- Health statement signed by a physician within 365 days, updated if >6 month break.
- Criminal Background Checks:
- Criminal background check within 45 days prior to start, including international searches if applicable.
- Massachusetts CORI check within 30 days prior to start.
- National sex offender registry check within 30 days prior to start, updated annually.
- Exclusion checks (OIG, GSA, MassHealth, NH Medicaid) within 30 days prior to start and annually.
- Respirator Medical Clearance Attestation: Required if fit test is needed.
- Education Verification: Verified through background check company.
- Massachusetts SSN Trace: Must be run with criminal check covering last 7 years.
- Confirmation of Assignment/POI Form: Required for all initial assignments and extensions.
Additional Information
- No religious exemptions for fit testing or influenza vaccine.
- Positive TB test requires negative chest x-ray documentation.
- Travel within the home health geographic area is required.
