Description

Job Opportunity: Radiology / Cardiology (Travel/Contract)

Location: San Angelo, Texas
Contract Length: 13 weeks (approximately 3 months)
Start Date: 12/15 strongly preferred
Schedule: Monday - Friday Evenings with call, includes holidays as scheduled


Licensure & Certification Requirements

  • ARRT or ARMRIT accepted
  • If only ARRT R, must have Texas radiology license
  • If ARRT MR, Texas radiology license NOT required

Required Certifications (AHA and ARC accepted where noted)

  • ACLS (Advanced Cardiac Life Support)
  • BLS (Basic Life Support)
  • PALS (Pediatric Advanced Life Support)
  • NRP (Neonatal Resuscitation Procedure) as applicable
  • CHEMO (Chemotherapy) as applicable
  • TNCC (Trauma Nursing Core Course) as applicable
  • EFM (Electronic Fetal Monitoring) as applicable
  • NIH Stroke Certification as applicable (required for ICU and PCU)
  • ENPC as applicable
  • State License Verification within 30 days of start
  • NURSYS Verification within 30 days of start
  • Tech Cert Verification - National within 30 days of start
  • Orientation Safety Manual Acknowledgement (annual update)
  • Pass/Fail Testing (EKG/Medication) during orientation

Background & Compliance

  • 7 Year Background Check required within 30 days of start (includes statewide, counties, federal, national, education verification, and international search if applicable)
  • EPLS/GSA/SAM and HHS/OIG checks required within 30 days of start
  • Education Verification required
  • Legal name must match on all documentation (license, ID, social security card, certifications)
  • Annual background check renewal required for Aya candidates

Health & Immunization Requirements

  • Physical Exam within 1 year of start (must be completed within 72 hours of contract signing)
  • Fit Mask Test within 6 months prior to start (annual updates required)
  • Negative TB test or Chest X-Ray within 1 year (or annual TB Questionnaire for positive responders)
  • Tetanus/Diphtheria/Tdap vaccine within 10 years
  • Rubeola (Measles), Mumps, Rubella: Positive titer or 2 vaccines required; boosters if negative/equivocal
  • Hepatitis B: Positive titer or series with declination options
  • Varicella: Positive titer, 2 vaccines, or proof of disease; boosters if negative/equivocal
  • Drug Testing required within 30 days (includes multiple substances)
  • Flu Seasonal Vaccination or facility-specific declination during flu season (10/31-3/31)
  • COVID-19 initial vaccine series required (boosters not required)

Documentation & Forms (Must be completed prior to start)

  • I9 or Attestation
  • Government Issued Photo ID (Driver’s License/State ID)
  • Social Security Card
  • Photo for Badge (passport-style with solid background)
  • Facility-specific forms including:
    • Non Employee Information Form
    • Vaccine Exemption Form (if applicable)
    • Applicant Health Record Form
    • Code of Conduct and Compliance Form
    • Compact- Temporary License Declaration of Residency Form
    • Confidentiality Statement Form
    • Information System Security Agreement Form
    • Non-Employee Orientation Update Form

Additional Requirements

  • 2 References within 3 years of start
  • Employment Application/Resume required
  • Skills Checklist (annual update)
  • Medication Test/Pharmacology Competency Exam as applicable
  • Competency Exam as applicable
