Home Health Physical Therapist (Travel/Contract)
Location: Radford, Virginia
Specialty: Therapy/Rehabilitation
Contract Length: 13 weeks
Position Overview
Seeking a Home Health Physical Therapist (DPT or Master's required) to provide personalized home care to patients recovering from illness, injury, or surgery. The role involves delivering expert care directly to patients' homes, supporting their recovery in familiar surroundings.
Schedule
- Shift: Monday to Friday, 8:00 AM – 4:30 PM (5 days a week, 8-hour shifts)
- Call Off Policy: Facility may call off 1 shift per 2 weeks
- Floating: Potential to float to a nearby region (~40 miles away) 2-3 times per week (mileage reimbursed)
Required Licenses & Certifications
- Virginia Physical Therapy license required (pending license accepted; can apply immediately after offer)
- Basic Life Support (BLS) certification required
- Additional specialized certifications preferred but not required
Required Clinical Skills
- Assessment: Adaptive/assistive devices, environmental barrier identification, vital sign monitoring
- Orthopedics: Ankle sprain, gait training, hip fractures/replacement, knee injuries/surgery, lower extremity amputations, post shoulder repair, post spinal surgery
- Documentation: Assessment, discharge, evaluation, goal setting (long/short term), plan of care, time management
- Cardiopulmonary: Breathing techniques, COPD, CHF, CABG, cystic fibrosis, emphysema
- Patient Populations: Cardiopulmonary, geriatrics, neurology, orthopedics, cognitive disorders, dementia/Alzheimer's
- Billing Knowledge: Commercial insurance, Medicaid, Medicare A/B, private pay, worker’s compensation
- Home Health experience: Required
Unit Details
- Based out of home care office in New River Valley
- Floating to Wytheville area 2-3 times per week (approx. 40 miles away)
Compliance & Documentation Requirements
- I9 or attestation required
- Driver’s License/State ID with address within 100 miles of facility required within 48 hours of signing
- E-Verify or attestation required
- Background Check: 7-year history required within 1 year of start; must cover all locations lived/worked in past 7 years
- Education Verification: Highest earned degree verification required
- Physical Exam: Within 1 year of start, confirming freedom from communicable diseases
- Drug Testing: Negative 10-panel drug screen within 30 days of start
- TB Testing: Negative PPD, QGold, or TSpot within 1 year; if positive, clear chest X-ray and annual questionnaire required
- Immunizations: Proof of immunity or vaccination for measles, mumps, rubella, hepatitis B, varicella
- Fit Testing: Required within 1 year and updated annually (not required for students)
- References: Two clinical references required
- Employment Application/Resume: Required
- Skills Checklist: Must be updated annually
Additional Notes
- EPIC charting experience preferred
- Accepts first-time travelers
- No radius rule for locals (but driver’s license address must be within 100 miles)
- Orientation and compliance paperwork must be completed prior to start
- Facility-specific safety and compliance training required
