Job Opportunity: Home Health Physical Therapist (PT)
Location: Mount Angel, Oregon (serving Salem and surrounding areas including South Salem, West Salem, Corvallis, Albany, Sweet Home, Lebanon)

Contract Length: 13 weeks
Job Details
- Start Date: ASAP
- Shift: 8-hour day shifts (No 4x10 or 3x12 shifts available)
- Patient Ratio: 1:1 (May visit up to 6 patients per day; 30 productivity points standard)
- Experience Required: Minimum 1 year of skilled home health experience
- First-Time Travelers: Accepted
- Weekend Requirement: No weekends required
- Certifications Required: BCLS (Basic Cardiac Life Support)
- On-Call: Not required
- Floating: Not required
- Block Scheduling: Not accommodated
- Locals Accepted: Yes, within 50-75 mile radius at same rate
- Pending License: Accepted
- Guaranteed Hours: Facility may call off up to one shift per pay period (every two weeks)
- Transportation: Traveler must have their own transportation
- Travel Radius: System groups clients to minimize drive time; occasional max radius ~60 miles (60-90 minutes total drive time) per day
- COVID-19 Vaccination: Primary series plus one booster required
- Documentation: OASIS documentation required

- Attire: Business casual; scrubs allowed but not required; no jeans
- Charting System: EPIC Remote Client (laptop) and Rover app (phone) for documentation and OASIS
- Parking: Free
- Mileage Reimbursement: IRS standard
Required Credentials & Compliance
- Background Checks:
- Must be within 45 days of start, covering all locations lived/worked in past 7 years
- Includes SSN trace, death master index, nationwide and county checks, sex offender registry, FACIS Level 3, education verification, motor vehicle record (MVR), and Oregon Abuse/Neglect search if applicable
- Returning candidates with less than 30-day gap may not require new background check
- Certifications:
- BCLS required
- Other certifications (ACLS, PALS, NRP, etc.) as applicable per clinical track
- Licenses:
- State professional license required and must be verified
- NURSYS verification for RNs/LPNs as applicable
- Health Requirements:
- Physical exam within 1 year of start, signed by MD, NP, PA, or DO
- Fit mask testing required (N95 or equivalent), updated annually
- Immunizations: TDAP (within 10 years), MMR (measles, mumps, rubella), Hepatitis B (vaccination or declination), Varicella, Flu vaccine (seasonal, declination accepted), COVID-19 vaccine (primary series plus booster required)
- Negative TB test or chest x-ray within 12 months, updated annually
- 10-panel drug screen within 90 days of start (no instant tests)
- References:
- Minimum 2 references covering 1 year of employment in a comparable specialty/setting within last 3 years
- Employment Application:
- Must include at least last 5 years of employment with month/year format and explanation of gaps or overlaps
Training & Competency Requirements
- Competency Exams: As applicable by profession
- Medication/Pharmacology Competency: Required if applicable
- OASIS Documentation: Required
- WorkRamp Modules:
- Completion of required Providence Health and Services WorkRamp training modules prior to start, including core, specialty, and regulatory modules as applicable
- Epic Training:
- Completion of EPIC training modules required within a week of orientation
Additional Notes
- Traveler may be required to float to another Providence facility within a reasonable distance (float policy acknowledgment required)
- Facility requires primary series COVID vaccination plus one booster
- Traveler must have reliable transportation due to travel between patient homes
- Facility groups patient visits to minimize driving time but expect occasional long drives (up to 60 miles)
- Facility cannot accommodate alternative shift patterns such as 4x10 or 3x12
