Job Description

• Acute care adult hospital case management
• Orientation to be completed at a different facility before transitioning to the primary hospital
• Social workers float to different units except trauma
• Work every third weekend
• Patient ratios:

  • Complex cases: 1 Social Worker paired with another RN managing 8-9 patients
  • Discharge planning coverage: 1 Social Worker managing 25-30 patients
• Documentation systems used: MIDAS/Meditech, Nate, Navihealth, Care Team Assignment
• Patient experience requirements include knowledge of hospital discharge planning/execution and familiarity with MDR (Multidisciplinary Rounds) or IDT (Interdisciplinary Team)
