Job Description

Provide physical therapy services to patients in their homes according to their plan of care. Requires specialized knowledge, judgment, and skill based on psychological, biological, and social service principles. Operate within established organizational and departmental policies.

Minimum Requirements

• Bachelor’s degree in Physical Therapy from an accredited program (Master’s or Doctorate preferred)
• Current licensure and registration as a Physical Therapist in Massachusetts
• Current American Heart Association BLS certification
• Minimum 1 year of physical therapy experience in a healthcare facility; home care experience required
• Strong verbal and written communication skills and excellent interpersonal skills
• Valid driver’s license, good driving record, and reliable transportation

Required Credentials and Compliance for Onboarding

• BLS Certification: AHA preferred; Red Cross accepted
• Professional License Verification: Primary source verification required; MA Conditional Licenses not accepted
• Fit Test: N95 respirator fit test within 365 days of start date, updated annually
• Driver’s License/State ID: Valid with signature if applicable
• Photo: Recent color photo (6 months), plain background, no filters or eyeglasses
• Motor Vehicle Record (MVR) Check: Within 45 days of start; required for home care roles
• Immunizations:

  • MMR (measles, mumps, rubella) vaccination or proof of immunity
  • Influenza vaccine required during flu season (Sept 15 - April 30), no exemptions
  • TB test (skin or blood test) within 3 months of start, updated if >6 month break
  • Health statement signed by MD/DO/NP/PA within 365 days, updated if >6 month break
• Criminal Background Checks:

  • National and Massachusetts CORI checks within 30-45 days of start, updated if >6 month break
  • SSN Trace and Exclusion Checks (OIG, GSA, Medicaid lists) within 30 days of start and annually
• References: Two recent references, including at least one supervisor or above
• Skills Checklist: Required for submission
• Resume: Past 7 years of work experience with city and state included
• Additional: Respirator Medical Clearance attestation if fit test required; confirmation of assignment form upon acceptance

Preferred / Optional

• Hepatitis B vaccination or proof of immunity (strongly preferred)
• Tdap vaccine within 10 years (preferred)
• Positive PPD with negative chest X-ray documentation if applicable

This position requires compliance with all credentialing, immunization, and background check policies prior to start.
