Home Health Physical Therapist (Travel/Contract)

Position Overview

Seeking a Home Health Physical Therapist (DPT or Master's required) to provide personalized home care to patients recovering from illness, injury, or surgery. The role involves delivering expert care directly in patients' homes to support recovery in a familiar environment.

Schedule

• Shift: Monday to Friday, 8:00 AM – 4:30 PM (5 days a week, 8-hour shifts)
• Charting: EPIC experience preferred
• Call Off Policy: Allowed 1 shift off per 2 weeks
• Floating: Potential to float to Wytheville area 2-3 times per week (approx. 40 miles from base office)

Licenses & Certifications

• Virginia Physical Therapy license required (pending license accepted; can apply immediately after offer)
• BLS (Basic Life Support) required
• Additional specialized certifications preferred but not required

Required Clinical Skills

• Assessment: Adaptive and assistive devices, environmental barrier identification, vital sign monitoring
• Orthopedics: Ankle sprain, gait training, hip fractures/replacement, knee injuries/surgery, lower extremity amputations, post shoulder repair, post spinal surgery
• Documentation: Assessment, discharge, evaluation, goal setting (long/short term), plan of care, time management
• Cardiopulmonary: Breathing techniques, COPD, CHF, CABG, cystic fibrosis, emphysema
• Patient Populations: Cardiopulmonary, geriatrics, neurology, orthopedics, cognitive disorders, dementia/Alzheimer's
• Billing Knowledge: Commercial insurance, Medicaid, Medicare A/B, private pay, worker's compensation
• Home Health experience: Required

Unit Details

• Based out of Carilion Clinic Home Care - New River Valley
• Floating to Wytheville area 2-3 times per week (mileage reimbursed)

Required Documentation & Compliance

• I9 or attestation
• Driver’s License/State ID with address within 100 miles of facility (required within 48 hours of signing)
• E-Verify or attestation
• Background Check: 7-year history, including all locations lived/worked in past 7 years
• Education Verification: Highest earned degree verification required
• Physical Exam: Within 1 year, free from communicable diseases
• Fit Mask Test: Within 1 year, updated annually (not required for students)
• Negative TB test or chest X-ray within 1 year, updated annually
• Immunizations: Measles, Mumps, Rubella, Hepatitis B, Varicella (positive titer or vaccination)
• Drug Testing: Negative 10-panel within 30 days of start
• References: 2 clinical references required

• Skills Checklist: Updated annually
• Compliance and orientation paperwork required prior to start

• VA State Police Search completed onsite on first day of orientation for home health candidates
