Description

Registered Nurse (Rehab RN - Medical Surgical/Rehab)

Location: Sylvania, Ohio
Employment Type: Travel/Contract
Contract Length: 9 weeks


Job Overview

  • Float required within service line and Med Surg, floating between multiple facilities as needed.
  • Weekend coverage every third weekend required.
  • Patient population: Adults and Geriatrics.
  • Unit: Rehab Unit (Medical Surgical/Rehab)
  • Patient diagnoses include: CVA, TBI, SCI, Neuro, Ortho, Amputee, Cancer/Cardiac, Debility.
  • No vents or telemetry equipment used.
  • Work setting primarily Med/Surg.
  • Patient ratios: Days 1:5, Nights 1:6, Weekends 1:5-6.
  • Guaranteed hours: 36 per week.

Experience & Skills Required

  • Minimum 1 year of experience required.
  • First-time travelers accepted but cannot be AO (approval needed by Unit Manager).
  • Previous charge nurse experience preferred.
  • EPIC charting experience required.
  • Patient ratio experience of 1:5 required.

Clinical Skills & Competencies

  • Acute inpatient rehab experience.
  • Blood glucose monitoring.
  • Feeding devices/adaptive equipment, feeding tube care and management (NG tube insertion, PEG tube feeding).
  • Swallowing precautions, thickened liquids.
  • Foley catheter insertion/removal.
  • Blood administration.
  • Care and management of ports.
  • IV starts.
  • Medication knowledge: Anti-coagulants, anti-seizure, diuretics.
  • Neurological assessments: CVA, neuromuscular disease, Parkinson’s, post-craniotomy, seizure disorders, spinal cord injury, stroke scale assessment, traumatic brain injury.
  • Assistive devices and mobilization techniques.
  • Pulse/CMS checks.
  • Suture/staple removal.
  • Total joint replacements.
  • Respiratory care: assessment of breath sounds, incentive spirometry, pulse oximetry, suctioning (oral and nasotracheal), supplemental oxygen delivery, tracheostomy care and suctioning.
  • Infection prevention and isolation precautions.
  • Fall risk assessment.
  • Pain assessment and management.
  • Patient teaching and diabetic education.
  • Pressure ulcer assessment/prevention and wound care.
  • Familiarity with National Patient Safety Goals/Core Measures.

Certifications Required

  • ACLS (AHA provider, instructor, or RQI accepted)
  • BLS (AHA provider, instructor, or RQI accepted)

Work Conditions & Additional Requirements

  • No on-call required.
  • Floating within scope to meet facility needs across multiple locations.
  • No local travelers within 50 miles accepted.
  • Return to office (RTO) restrictions: Greater than 7 days off requires management approval.
  • Scrub color: Navy.
  • Charting system: EPIC.
  • Parking: Free.

Compliance & Documentation Requirements

  • 7-year background check within 12 months of start, updated annually.
  • EPLS/GSA/SAM and HHS/OIG checks required annually.
  • Fingerprinting required for certain units; Ohio BCI and FBI fingerprint results must be within 12 months of start and updated annually.
  • Education verification required.
  • Physical exam within last 12 months (latex-free PPE available).
  • N95 fit test within 12 months, updated annually (KN95 not accepted).
  • Immunizations and titers required or approved exemptions for:
    • Tetanus/Diphtheria/Tdap (within 10 years)
    • Measles, Mumps, Rubella (MMR)
    • Varicella
    • Hepatitis B (series, positive titer, or declination)
    • Flu vaccine or approved exemption (seasonal requirement)
  • Negative TB test or chest X-ray within 3 months, updated annually.
  • Negative 10-panel drug screen within 12 months.
  • References: 2 professional references within 2 years, including at least one from a leader (Charge RN or above).
  • Skills checklist required for all candidates.

Patient Care Focus

  • Rehab unit with 3 hours of medical rehab daily.
  • No ventilators or telemetry.
  • Focus on adult and geriatric patients with complex rehab needs.

This position is ideal for RNs with acute inpatient rehab experience who are comfortable floating within a service line and working with a diverse adult and geriatric patient population in a rehab/med-surg setting. Strong EPIC charting skills and required certifications (ACLS, BLS) are mandatory. Weekend coverage every third weekend is required.
