Job Opportunity: Home Health Occupational Therapist (OT)
Location: Medford, Oregon and Surrounding Areas
Specialty: Therapy/Rehabilitation
Employment Type: Travel/Contract
Contract Length: 13 weeks
Job Details
- Start Date: ASAP
- Shift: 8-hour day shifts (No 4x10 or 3x12 shifts available)
- Patient Ratio: 1:1 (May visit up to 6 patients per day; 30 productivity points standard)
- Experience Required: Minimum 1 year of skilled home health experience
- First-Time Travelers: Not accepted
- Weekend Requirement: No weekends required
- Certifications Required: BCLS (Basic Cardiac Life Support)
- On-Call: Not required
- Floating: No floating between units required
- Block Schedule: Not accommodated
- Locals Accepted: Yes, within 50-75 mile radius (considered local)
- Pending License: Accepted (confirm restrictions with hiring manager)
- Guaranteed Hours: Facility may call off up to one shift every two weeks
- Transportation: Traveler must have own transportation
- Travel Radius: System groups clients to minimize drive time; occasional max radius 50+ miles
- COVID-19 Vaccination: Primary series plus one booster required
- Charting System: EPIC/Oasis (Oasis experience required)
- Attire: Any color scrubs
- Parking: Free
- Mileage Reimbursement: IRS standard
Required Qualifications & Documentation
- Licenses & Certifications:
- BCLS required
- Other certifications (e.g., ACLS, PALS, NRP) as applicable per clinical track
- State professional license verification
- Background Checks:
- Comprehensive background check within 45 days of start, covering all locations lived/worked in past 7 years
- Includes SSN trace, death master index, nationwide and county checks, motor vehicle record (MVR), and state-specific abuse/neglect searches as applicable
- Education verification required
- Health Requirements:
- Physical exam within 1 year of start, signed by MD, NP, PA, or DO
- Immunizations/vaccinations: COVID-19 (primary + booster), Flu (seasonal), TDAP, MMR (Measles, Mumps, Rubella), Varicella, Hepatitis B (or signed declination)
- Negative TB test or chest x-ray within 12 months
- Drug screening: 10-panel within 90 days of start (no instant tests)
- Respirator clearance (RespSafety) required for travel candidates
- Fit mask testing required for travel candidates (annual update)
- References & Employment History:
- Minimum 2 references covering 1 year of employment in a comparable specialty within last 3 years (6 months for CNAs)
- Employment application covering at least last 5 years (7 years for APN/subcontractors)
- Competency Exams:
- Medication test/pharmacology competency as applicable
- Specialty-specific competency exams if required
- Facility-Specific Training:
- Completion of required WorkRamp modules (Core, Specialty, Regional, and Annual Regulatory) prior to start
- EPIC training modules required within a week of orientation
- Other:
- Traveler must provide a passport-style badge photo with neutral background
- Must complete any required state-specific searches and attestations (e.g., WA multi-state licensed nurse attestation if applicable)
Additional Notes
- This is a home health position; the facility does a good job grouping clients to minimize driving time but expect occasional travel up to 50+ miles.
- The facility cannot accommodate alternative shift patterns such as 4x10 or 3x12.
- Locals within a 50-75 mile radius are considered local and accepted at the same rate.
- The facility may call off up to one shift every two weeks, so guaranteed hours are not absolute.
- COVID-19 vaccination including booster is mandatory.
- Oasis charting experience is required; candidates without Oasis experience should disclose this upfront.
This position is ideal for experienced home health occupational therapists who meet the certification and documentation requirements and are comfortable with travel within the Medford, OR area.
