Job Opportunity: Home Health Physical Therapist (Travel/Contract)

Location: Portland, Oregon
Specialty: Therapy/Rehabilitation


Job Details

  • Setting: Home Health (East Portland and surrounding areas)
  • Shift: 8-hour day shifts (No 4x10 or 3x12 shifts available)
  • Patient Load: 1:1 ratio; may visit up to 6 patients per day
  • Productivity Standard: 30 productivity points per day
  • Experience Required: Minimum 1 year of skilled home health experience
  • First-Time Travelers: Accepted
  • Weekend Requirement: No weekends required
  • On-Call: Not required
  • Floating: No floating between units required
  • Block Scheduling: Not accommodated
  • Locals: Accepted within 50-75 mile radius
  • Pending License: Accepted
  • Guaranteed Hours: Facility may call off up to one shift every two weeks
  • Transportation: Traveler must have own transportation
  • Travel Radius: Up to 60 miles per day (60-90 minutes total drive time)
  • COVID-19 Vaccination: Primary series plus one booster required
  • Documentation System: EPIC Remote Client (laptop), Rover app (phone), and OASIS required
  • OASIS Experience: Must be noted if not experienced
  • Attire: Business casual; scrubs allowed but no specific color required; no jeans
  • Parking: Free
  • Mileage Reimbursement: IRS standard

Contract Length

  • Duration: 13 weeks

Required Certifications

  • BCLS (Basic Cardiac Life Support) - Required
  • Other certifications may be required based on clinical track and specialty (e.g., ACLS, PALS, NRP) but BCLS is mandatory for this role.

Licensure and Background Requirements

  • Must hold a valid license to practice in Oregon (and Washington if working in border units)
  • Background check within 45 days of start, covering all locations lived/worked in past 7 years
  • State-specific searches and verifications as applicable (e.g., Oregon Abuse/Neglect, Washington WATCH Search)
  • Education verification required
  • Drug screening (10 panel) within 90 days of start
  • References: Minimum 2 supervisor references covering 1 year of employment in a comparable specialty/setting within the last 3 years
  • Employment application covering at least last 5 years

Health and Safety Requirements

  • Physical exam within 1 year of start, signed by MD, NP, PA, or DO
  • Fit mask testing required (annual update)
  • Immunizations and documentation required:
    • COVID-19 vaccination or declination
    • Flu vaccine during flu season or declination
    • Tetanus/Diphtheria/Tdap within 10 years or declination
    • Negative TB test or chest X-ray within 12 months, updated annually
    • MMR (Measles, Mumps, Rubella) vaccination or positive titer or declination
    • Hepatitis B vaccination or signed declination
    • Varicella vaccination or positive titer or declination

Training and Competency Requirements

  • Completion of Providence Health and Services WorkRamp modules prior to start, including:
    • Core modules for Home Health caregivers
    • Specialty-specific modules as applicable
    • Annual regulatory requirements
  • Epic training modules required and must be completed within a week of orientation
  • Competency exams as applicable by profession
  • Medication test/pharmacology competency exam as applicable

Additional Notes

  • Traveler may be required to float to another Providence facility within a reasonable distance (float policy acknowledgment required)
  • Traveler must comply with all Providence Health and Services policies and procedures
  • Facility uses EPIC and Rover for charting and documentation
  • Facility requires a passport-style badge photo for identification
