Job Description

  • Support an innovative care delivery model called Behavioral Health Home Hospital (BHH), providing an alternative to inpatient psychiatric hospital admission by delivering specialized psychiatric care in patients' homes.
  • Provide quality clinical services to patients in community settings, including in-person visits at patients' homes or residences and remote care.
  • Manage up to 4 patients at a time, delivering hospital/inpatient-level psychiatric treatment in the community.
  • Collaborate with interdisciplinary teams to address psychosocial, environmental, age-specific, and cultural issues to maximize patients' emotional, social, and physical well-being.
  • Utilize clinical social work techniques for consultation, assessment, treatment interventions, and advocacy.
  • Participate in a research trial aimed at broadening treatment options for patients with active acute psychiatric conditions.
  • Anticipate working about an hour a day, schedule depending on caseload.
  • Position start date: January 5 (aligned with trial start).

Required Credentials & Compliance

  • Professional Licensure: Valid LCSW license with primary source verification; MA Conditional Licenses not accepted.
  • Photo ID: Recent (within 6 months) professional badge photo meeting specific guidelines (no selfies, plain background, no eyeglasses).
  • Health Requirements:
    • N95 Fit Test within 1 year, updated annually (no onsite testing).
    • Documentation of MMR vaccination or immunity proof.
    • Influenza vaccine required during flu season (Sept 15 - April 30), no exemptions.
    • TB screening within 3 months of start date (skin test or blood test), updated if break >6 months.
    • Physician health statement within 1 year, updated if break >6 months.
    • Hepatitis B vaccination or immunity strongly preferred.
    • Tdap vaccine within 10 years preferred.
  • Background Checks:
    • Criminal background check within 45 days prior to start, including international searches if applicable.
    • Massachusetts CORI background check within 30 days prior to start.
    • National sex offender registry check within 30 days prior to start, updated annually.
    • Exclusion checks (OIG, GSA, MassHealth, NH Medicaid) within 30 days prior to start and annually.
    • Social Security Number trace for past 7 years addresses/employment/school.
  • Documentation for Submission:
    • Resume including past 7 years of work experience with city and state.
    • Two recent references (minimum one supervisor or above).
    • Skills checklist.
    • Candidate submission coversheet.
  • Other:
    • Respirator Medical Clearance attestation (part of fit test documentation).
    • Confirmation of assignment/POI form upon acceptance and for any start date changes.
    • Education verification through background check company.
