Description
Physical Therapist Outpatient - Orleans, Massachusetts
Employment Type: Long-Term Assignment (LTA)
Contract Length: 13 weeks
Job Summary
- Assist patients in reaching maximum physical performance while adjusting to disabilities.
- Restore patient physical functioning by interpreting physical therapy evaluations and test results.
- Determine physical therapy treatment goals and plans.
- Administer manual exercises.
- Instruct, encourage, and assist patients in performing physical activities and using assistive/supportive devices.
Required Credentials and Compliance for Onboarding
Certifications
- Basic Life Support (BLS) required (AHA, Red Cross, CHSF, or Military accepted; AHA preferred).
Licenses
- Primary source verification of professional license/certification required within 30 days of start.
- Massachusetts Conditional Licenses not accepted.
Professional Requirements
- Nurse Aide Registry check (within 30 days of start).
- Orientation summary completion.
- Criminal background check within 45 days prior to start and updated if >6 month break.
- Massachusetts CORI background check within 30 days prior to start and updated if >6 month break.
- Resume including past 7 years of work experience (required for submission).
- Two recent references (minimum one supervisor or above in skillset).
- Skills checklist (required for submission).
- Confirmation of assignment/POI form for initial assignments and extensions.
- Education verification of highest level achieved.
- Social Security Number trace with criminal check.
- National sex offender registry check within 30 days of start and updated annually.
- Exclusion checks (OIG, GSA, MassHealth, NH Medicaid) within 30 days of start and annually.
Personal Health History
- Documentation of MMR vaccination or proof of immunity.
- Influenza vaccine required during flu season (Sept 15 - April 30); no exemptions allowed.
- TB test documentation within 3 months of start and updated if >6 month break (skin test or blood test).
- Health statement signed by MD/DO/NP/PA within 365 days of start and updated if >6 month break.
- Respirator Medical Clearance attestation if fit test required.
- Hepatitis B vaccine or immunity strongly preferred.
- Tdap vaccine within 10 years preferred.
- Positive PPD requires negative chest x-ray documentation (optional).
- TB symptom survey if positive PPD (optional).
This position requires compliance with all credentialing, background checks, and health requirements prior to and during employment.
