Job Opportunity: Digital Acute Care Registered Nurse (SFMC - On Call)

Location: Peoria, Illinois

Position Summary

• Provide virtual and in-person nursing care to adult patients through telehealth technology and home visits within a 30-minute radius.
• Deliver proactive monitoring, prevention, early intervention, and ongoing care to optimize patients' quality of life at home.
• Use evidence-based care and clinical decision support to guide nursing practice.
• Collaborate with healthcare team members, patients, and families to promote quality care.
• Act as a patient advocate and resource for healthcare providers and community resources.
• Demonstrate service excellence and accountability, balancing cost-effective and efficient care delivery.

Required Qualifications

• Education: Associate Degree in Nursing (Bachelor’s degree preferred).
• Licenses/Certifications:

  • Registered Nurse (RN) license.
  • American Heart Association BLS certification.
  • Valid current Illinois state driver’s license.
• Experience:

  • Minimum 1 year recent RN experience in adult acute care population.
  • Minimum 2 years experience in roles requiring effective communication and collaboration with healthcare professionals.
• Skills:

  • Excellent interpersonal and communication skills.
  • Proficient computer skills, including Microsoft software.
  • Strong analytical and problem-solving abilities with attention to detail.
• Other Requirements:

  • Proficiency with electronic EMR and communication platforms.
  • Transportation for travel to/from patients’ homes (fleet car provided).
  • Must meet motor vehicle report and insurance coverage compliance.

Contract Length

• 13 weeks

Additional Information

• Orientation includes 12 non-billable hours.
• Local clinicians within 50 miles may have adjusted rates.
• Call off policy allows 12 hours off every 13 weeks without penalty.
• Drug-free workplace: negative drug screen required for all panels.
• Must provide active Illinois nursing license or proof of application initiation.
