Job Description

• Position: Physical Therapist
• Work setting: Inpatient and outpatient
• Licensure: Washington State Physical Therapist license required
• Certifications:

  • BLS (Basic Life Support) required
  • ACLS, PALS, and other certifications may be required depending on unit (refer to facility/unit specifics)
• Experience: Minimum 1 year of experience in specialty (default to higher if conflicting info; here only general experience implied)
• Prior travel experience: Not explicitly required

Required Credentials & Documentation

• Copy of I9 or attestation
• 10-year criminal background check including:

  • SSN Trace
  • Alias/Name changes
  • Statewide searches for all states lived/worked in last 10 years
  • National criminal, sex offender checks
  • Education verification
• EPLS/GSA/SAM and HHS/OIG checks within 30 days of start
• Washington State Patrol (WSP/WATCH) search within 30 days of start
• Education verification and diploma copy (if applicable)
• Current Washington State license verification within 30 days of start
• Photo for badge (headshot on plain white background)
• Signed Code of Conduct and confidentiality agreements
• Employment application and 3 recent employment references

Health & Safety Requirements

• Physical exam (signed by physician within 1 year of start)
• Fit mask testing (N95) performed onsite; required for certain units
• Immunizations and titers required or declinations accepted for:

  • Tetanus/Diphtheria/Tdap
  • Measles (Rubeola)
  • Mumps
  • Rubella
  • Hepatitis B
  • Varicella
• Negative TB test (QGold or T-SPOT) within 30 days of start; chest X-ray if positive
• TB questionnaire annually
• Drug screen (10 panel) within 30 days of start; no instant tests accepted
• Seasonal flu vaccination or declination required by November 1

Additional Notes

• Facility requires completion of orientation and fit testing onsite
• Various certifications may be required depending on unit assignment (e.g., ACLS, PALS, NRP, TNCC)
• Professional Assault Crisis Training (ProAct) required within 6 months for Behavioral Health RNs (not applicable here)
• Candidate must complete onboarding and new employee paperwork prior to start

This position is ideal for a licensed Physical Therapist with inpatient/outpatient experience seeking a 13-week travel contract in Washington state.
