Job Opportunity: Home Health Physical Therapist (Travel/Contract)

Location: Portland, Oregon


Position Details

  • Specialty: Therapy/Rehabilitation (Physical Therapy)
  • Employment Type: Travel/Contract
  • Contract Length: 13 weeks
  • Start Date: December 1, 2025
  • Shift: 8-hour day shifts (No 4x10 or 3x12 shifts available)
  • Patient Ratio: 1:1 (up to 6 patients per day; 30 productivity points standard)
  • Years of Experience Required: Minimum 1 year of skilled home health experience (highest requirement found)
  • Prior Travel Experience: Not explicitly required

Certification Requirements

  • BCLS (Basic Cardiac Life Support): Required
  • Other certifications (ACLS, PALS, NRP, etc.) may be required depending on specialty and clinical track but BCLS is mandatory for this role.

Background & Compliance Requirements

  • Background check within 45 days of start, covering all locations lived/worked in past 7 years
  • Various state-specific background and abuse/neglect searches as applicable
  • Education verification required
  • Drug screening: 10-panel drug screen within 90 days of start (no instant tests)
  • Physical exam within 1 year of start, signed by MD, NP, PA, or DO
  • Immunizations and health documents required:
    • TDAP (within 10 years)
    • Negative TB test or chest X-ray within 12 months
    • MMR (Measles, Mumps, Rubella) vaccination or positive titer
    • Hepatitis B vaccination or signed declination
    • Varicella vaccination or positive titer
    • Flu vaccine during flu season or declination
    • COVID vaccine or declination
  • Respirator fit testing required for travel candidates starting on or after 2/5/2024
  • References: Minimum 2 supervisor references covering 1 year of employment in a comparable specialty/setting within the last 3 years

Training & Competency Requirements

  • Completion of Providence Health and Services WorkRamp modules prior to start, including:
    • Core modules for Oregon Home Health professionals
    • Specialty-specific modules as applicable
    • Annual regulatory requirements
  • Competency exams and medication/pharmacology tests as applicable
  • Epic training modules required for charting system access

Additional Notes

  • Traveler may be required to comply with float policy within the Providence system, though this specific role does not require floating between units
  • Must comply with all state-specific licensing and attestation requirements, including multi-state licensure if applicable
  • Must be able to work within the specified travel radius and have reliable transportation
  • Facility can call off up to one shift per pay period (every two weeks)

This position is ideal for a Physical Therapist with at least 1 year of skilled home health experience who is comfortable working independently in a home health setting, has required certifications, and is willing to travel within the Portland, OR area. First-time travelers are welcome.
