Job Opportunity: Home Health Physical Therapist (Travel/Contract)

Job Details

• Start Date: ASAP
• Shift: 8-hour day shifts (No 4x10 or 3x12 shifts available)
• Patient Load: 1:1 ratio; up to 6 patients per day; 30 productivity points standard
• Experience Required: Minimum 1 year of skilled home health experience
• First-Time Travelers: Accepted
• Weekend Requirement: No
• On-Call: No
• Floating: No floating between units required
• Block Schedule: Not accommodated
• Locals Accepted: Yes, within 50-75 mile radius
• Pending License: Accepted
• Guaranteed Hours: Facility may call off up to one shift every two weeks
• Transportation: Traveler must have own transportation
• Travel Radius: Up to approximately 60 miles per day (60-90 minutes total drive time)
• Documentation System: EPIC Remote Client (laptop), Rover app (phone), and Oasis documentation required
• Oasis Experience: Required

Certifications Required

• BCLS (Basic Cardiac Life Support)
• Other certifications may be required based on clinical track but BCLS is mandatory for this role

Licensure & Background Requirements

• Must have valid state license or compact license accepted
• Background check within 45 days of start, covering all locations lived/worked in past 7 years
• Additional state-specific background checks and abuse/neglect searches as applicable
• Education verification required
• Drug screening (10 panel) within 90 days of start
• Physical exam within 1 year of start, signed by MD, NP, PA, or DO
• Immunizations and health documents required:

  • TDAP (within 10 years) or declination
  • Negative TB test or chest x-ray within 12 months
  • MMR (Measles, Mumps, Rubella) vaccination or positive titer
  • Hepatitis B vaccination or declination
  • Varicella vaccination or positive titer
  • Seasonal flu vaccine or declination (required during flu season)
  • COVID vaccine or declination

Additional Requirements

• Must complete Providence Health and Services required WorkRamp training modules prior to start (specific to Oregon region and home health specialty)
• Fit mask testing required for travel candidates (updated annually)
• References: Minimum 2 supervisor references covering 1 year of employment in a comparable specialty/setting within the last 3 years
• Employment application covering at least last 5 years required
• Skills checklist updated annually as applicable
