Job Opportunity: Physical Therapist (Rehabilitation Therapy)

Job Description

The Physical Therapist will collaborate with residents and healthcare providers to deliver physical therapy services aimed at maximizing residents' functional abilities and independence in daily activities.

Key Responsibilities

• Perform evaluative and therapeutic procedures under physician or healthcare provider orders.
• Evaluate residents and develop therapeutic plans of care with functional goals, frequency, duration, and modalities.
• Incorporate resident and caregiver input into therapy goals.
• Consult with physicians and healthcare team members regarding care plans and discharge needs.
• Monitor and adjust treatment plans based on resident progress.
• Document all interactions and treatments in the Electronic Medical Record clearly and accessibly.
• Recommend and instruct on appropriate assistive devices.
• Provide education to residents and caregivers, documenting teaching outcomes.
• Collaborate with Physical Therapist Assistants and other therapy professionals.
• Ensure compliance with Medicare, Medicaid, and other reimbursement regulations.
• Provide in-service education and maintain clinical skills through ongoing learning.
• Assist in developing new rehabilitation programs and services.
• Maintain awareness of professional issues and share relevant information with staff.
• Supervise assistant-level staff as appropriate, ensuring adherence to state and professional standards.
• Mentor new staff during orientation and ensure completion of competencies.
• Maintain cleanliness and infection control in clinical areas.
• Maintain required licenses, certifications, and complete annual compliance courses.

Requirements

Education & Licensure

• Bachelor’s degree required; Master’s or Doctorate preferred.
• Active Physical Therapist license in Rhode Island.
• Basic Life Support (BLS) certification preferred.

Experience

• Prior experience in long-term care or skilled nursing facility preferred.
• Years of Experience Required: Not explicitly stated; assume minimum 2 years based on typical requirements for this specialty.
