Facility & Unit Information
- Hospice census: ~100 patients total
- 21 beds inpatient (McClure Miller Respite House)
- 80 patients in home, Skilled Nursing Facility (SNF), Assisted Living Facility (ALF) settings
- Unit Specialty: Home Health Hospice
- Teaching Facility: Yes
- Teaching Hospital Experience: Not required
- Trauma Level: N/A
- Nurse-Patient Ratio:
- Home setting: 12-14 patients per nurse
- Inpatient (MMRH) day shift: 5-7 patients
- Inpatient overnight shift: up to 11 patients
- Ancillary Staff: CNA/PCT available (4 per shift)
- Advanced Practice Provider: Full-time Nurse Practitioner on unit
Job Details
- Employment Type: Contract (12 weeks)
- Shift Hours:
- Inpatient (MMRH): 7-7:30 (12-hour shifts)
- Home Hospice: 8 AM - 4:30 PM, Monday-Friday
- Rotation: Between home hospice and inpatient respite house
- Scheduling: No self-scheduling; weekend shifts required on rotation
- Call Requirements: Rare coverage needed, may cover on-call team
- Holiday Requirements: If needed
- Floating: Not currently required, but some cross-setting training expected
Patient Population & Care
- Patient Population: Terminally ill patients with prognosis of 6 months or less
- Age Range: Primarily adults, occasionally pediatrics in home setting
- Care Focus: Hospice/end-of-life care, complex symptom management, education
- Procedures & Protocols:
- Pain management including continuous infusion and palliative sedation
- Central line care (PORT/IVAD, PICC)
- Tracheostomy and wound care
- Ostomy and drain management
- Venipuncture for labs/specimens
- Equipment Used:
- CADD pumps, feeding pumps, suction units, BiPAP/CPAP, nebulizers, IV pumps, glucose and INR monitors
- Respiratory and anesthesia support: Not available 24/7 onsite
Experience & Certification Requirements
- Minimum Clinical Experience: 3-5 years
- Minimum Specialty Experience: 3 years in Home Health/Hospice
- Prior Travel Experience: Not required
- Certifications Required: BLS
- Orientation: 2-3 days, includes online onboarding and training on documentation system (Net Smart)
- Documentation System: Net Smart (Oasis not required for Hospice)

- Scrubs/Color: Business casual or non-hospital scrubs in non-institutional colors
- Shoes: Closed toe, supportive
- Jewelry: Limited, cannot interview with precautions
- Hair/Beard: Neat and tidy
- Nail Polish: Not allowed
Additional Information for Medical Social Worker Role
- Education: Master’s Degree in Social Work (MSW)
- Licensure: Vermont LMSW required
- Experience: 2+ years relevant experience
- Schedule: Monday to Sunday, all 8-hour shifts, no block schedules
- Cross Coverage: May be expected to cover adult home health, hospice, and community hospice as needed
- Transportation: Reliable personal transportation required due to lack of public transit and winter driving conditions (snow tires recommended)
- Employment History: Complete work history required including bed count, EMR, and facility type
