Job Opportunity: Home Health Physical Therapist (Travel/Contract)

Location: Manchester, New Hampshire
Specialty: Therapy/Rehabilitation (Physical Therapy)
Contract Length: 13 weeks


Job Summary

  • Home health physical therapy position
  • Manager will accept candidates with no prior home health experience
  • Minimum 1 year of experience required in specialty
  • Prior travel experience NOT required (first-time travelers accepted)
  • NH state license required; candidates with license pending may be accepted if license can be obtained by start date

Work Schedule & Details

  • Visits per day: Approximately 6
  • Shift: Monday to Friday, 8:00 AM – 4:30 PM (5x8)
  • Call: None
  • Float: None
  • Weekend: Occasional weekends as needed
  • Documentation system: Epic
  • Orientation: 10 hours
  • Scrubs: Any

Licensure & Certifications

  • Must have a valid New Hampshire license (temporary licenses accepted if valid by start date)
  • Required certifications (AHA provider cards accepted where applicable):
    • ACLS (Advanced Cardiac Life Support)
    • BLS (Basic Life Support)
    • PALS (Pediatric Advanced Life Support) – if applicable
  • Other certifications may be required depending on unit (e.g., Management of Assaultive Behavior, NIH Stroke Certification, CPI)
  • Verification of professional license required

Background & Compliance Requirements

  • Background check required within 30 days of start, including:
    • Social Security Verification
    • National Sex Offender Registry
    • National Criminal Search
    • FACIS Level 3
    • Education Verification
    • Criminal county/state searches (excluding NH counties; NH State Police search required instead)
    • Motor Vehicle Background Check (for those driving to patient homes)
  • Drug screen: 10-panel within 14 days prior to start (includes amphetamines, benzodiazepines, cocaine, fentanyl, opiates, etc.)
  • EPLS/GSA/SAM and HHS/OIG checks required within 30 days prior to start
  • SSA release and statewide releases (NH, MA if applicable) required
  • References: Two current professional references within last 3 years, including one supervisor reference
  • Employment application and skills checklist required

Health & Immunization Requirements

  • Physical exam within 1 year prior to start
  • Fit mask test annually (not required for PT outpatient candidates)
  • Tetanus/Diphtheria/Tdap vaccine within last 10 years (medical or religious exemptions allowed with documentation)
  • Negative TB test or chest x-ray within 1 year (2-step PPD or equivalent) plus TB questionnaire required
  • Immunity or vaccination documentation required for:
    • Rubeola (Measles)
    • Mumps
    • Rubella
    • Hepatitis B (declinations accepted with waiver)
    • Varicella (no history of disease accepted, only vaccination or titer)
  • Flu vaccination required annually during flu season (Oct 31 – Apr 1) with medical or religious exemptions allowed with documentation
  • Respiratory safety clearance questionnaire required prior to start

Additional Notes

  • Travelers who have ever been employed by any facility associated with this health system are not eligible
  • Proof of auto insurance required for those driving to patient homes
  • Facility requires photo for badge (passport style, jpeg)
  • Orientation safety manual acknowledgment and compliance attestation required
  • Pass/fail testing (e.g., EKG/Dysrhythmia) may be required depending on unit assignment
