Job Summary for Healthcare Recruiters and Workers

Position: LPN for Long-Term Care (LTC) / Skilled Nursing Facility (SNF) Unit
Location: New York (within an acute care setting)
Contract Length: Approximately 16 weeks (interpreted as weeks)
Unit Specialty: Long-term care / Skilled Rehab
Unit Size: 34 beds


Experience & Licensing Requirements

  • Minimum Clinical Experience: 1 year
  • Minimum Experience in Specialty: 1 year
  • Prior Travel Experience: Not required, but preferred
  • Licensing:
    • NY State license required or proof of application with applied date at time of submission
    • Note: NY licensing process takes 6-8 weeks, which will be considered for start dates/offers

Work Environment & Unit Details

  • Facility Type: 34-bed LTC unit situated within an acute care hospital
  • Patient Population: Varied diagnoses, age range 45-99+
  • Nurse-to-Patient Ratio: 7 patients per nurse (varies by shift)
  • Ancillary Staff Support:
    • CNAs/PCTs present on all shifts (staffing varies by shift)
    • Unit clerk/secretary available on day shift
    • Advanced Practice Provider (LTC Nurse Practitioner) on unit
  • Orientation: 2-3 days
  • Documentation System: SigmaCare and Epic
  • Medication System: SigmaCare
  • Floating: Required (may float to acute hospital as sitter if needed)
  • Charge Nurse: Not required
  • Scheduling:
    • Smart Square with staffing resource center
    • Shift hours as requested
    • Weekend requirement: every other weekend minimum
    • No call requirements
    • Holiday work as scheduled
    • No self-scheduling or block schedules

Patient Care & Equipment

  • Care Modality: Long-term care / Skilled nursing
  • Special Equipment: Basic LTC equipment only (no ventilators or invasive monitoring)
  • Respiratory Support: Available 24/7
  • Anesthesia Support: Not available on unit
  • Ventilator Management: Not applicable

Dress Code & Professional Appearance

  • Scrubs: Nursing navy or white (aides may wear what they have)
  • Shoes: Per dress code
  • Jewelry: Minimal, per dress code
  • Hair/Beard: Groomed or none to fit masks
  • Nail Polish: Per dress code

Additional Notes

  • IV Skills (IVS) will be offered
  • Work schedule: Monday through Sunday
  • Bi-weekly orientations on Mondays
  • No public transportation available; reliable personal or rental vehicle strongly recommended
  • Vehicles should have all-season or snow tires from November through April
  • Complete work history required for submission, including bed count, EMR, facility type, and explanations for employment gaps over 30 days
  • COVID and flu vaccination declinations accepted with proof or declination documentation

This position is ideal for LPNs with at least 1 year of clinical and specialty experience seeking a travel assignment in a long-term care setting within an acute care hospital environment.
