Job Opportunity: Physical Therapist Home Health

Location: Newton, Massachusetts
Employment Type: Long-Term Assignment (LTA)
Contract Length: 13 weeks

Job Summary

Provide physical therapy services to patients in their homes who have physical impairments, functional limitations, or disabilities due to injury, disease, or other causes. Patient ages range from birth to over 100 years. Collaborate with patients, families, physicians, and other allied health professionals to identify needs and coordinate care. Maintain thorough and timely electronic documentation and interdisciplinary communication according to agency policies and standards of practice.

Requirements

Licensure & Experience

• Must hold a valid Physical Therapist professional license in Massachusetts.
• Minimum 1 year of home care experience required.
• Ability to travel to assigned cases.
• Weekend rotations occur approximately once every 6 weeks.

Prior Travel Experience

• No explicit prior travel experience required.

Credentials & Compliance (Onboarding Requirements)

• Professional license verification (no conditional licenses accepted).
• Background checks including criminal, CORI, SSN trace, MVR, national sex offender, and exclusion checks.
• Submission of a recent professional photo (no selfies, plain background).
• Fit test for N95 respirator within 365 days of start date (no religious exemptions).
• Health documentation including:

  • TB test within 3 months of start date (skin test or blood test).
  • Influenza vaccine during flu season (no exemptions).
  • MMR vaccination or proof of immunity.
  • Health statement signed by a licensed provider within 365 days.
• Submission of references (2 recent, at least 1 supervisor or above).
• Skills checklist and candidate submission coversheet required.
• Education verification and resume covering past 7 years of work experience.
• Respirator medical clearance attestation if fit test required.

• Must comply with all agency policies and standards of practice.
• Documentation and credentialing must be current and updated as required.
• No conditional or temporary licenses accepted.
• No religious or medical exemptions for flu vaccine or fit testing.
