Description

Home Health Travel Nurse Opportunity

Position Details

  • Specialty: Home Health
  • Employment Type: Travel
  • Contract Length: 16 weeks (approximately 4 months)
  • Shift Hours: 8:00 AM – 5:00 PM
  • Weekend Requirements: 1 weekend per month with 2 compensatory days off during the week
  • Holiday Requirements: As needed
  • Call Requirements: None
  • Scheduling: Self-scheduling within department guidelines; no block schedules
  • Orientation: 2-3 days, bi-weekly on Mondays

Experience & Certifications

  • Required Specialty Experience: 3-5 years in Home Health (higher requirement overrides lower)
  • Prior Travel Experience: Required
  • Certifications Required: AHA BLS, OASIS experience
  • Preferred Certifications: OASIS proficiency
  • EMR Experience: Fluency in navigating multiple EMRs required; experience with NetSmart preferred

Caseload & Patient Population

  • Average Caseload: 22-28 clients per day
  • Visits: 6-7 units daily (routine visits, admissions, recertifications, discharges)
  • Patient Diagnoses: Cardiac, pulmonary, diabetes, wound care, post-surgery, orthopedics, neurological impairment, palliative care, dementia, CVA, SCI, COPD, Parkinson’s, ostomy, catheters, MVA, etc.
  • Age Range: 30-100 years

Work Environment & Equipment

  • Patient Care Modality: Adult Home Health
  • Nurse-Patient Ratio: Average caseload of 18-22 clients for case managers; equivalent to about 5 visits per day
  • Documentation System: Netsmart (training provided during orientation)
  • Special Equipment Provided: Laptop, iPhone, clinical supplies including wound care, catheters, trunk bin/visit bag
  • Dress Code: Business casual or any color scrubs; closed, non-skid shoes; minimal jewelry and nail polish; groomed hair/beard

Clinical Skills & Procedures

  • Strong clinical and assessment skills required
  • Unit-specific procedures include:
    • Phlebotomy
    • Catheter care
    • IV therapy
    • Coagulation monitoring (coag-u-check)
    • Port access
    • Wound vacs
    • TPN (Total Parenteral Nutrition)
    • Feeding tubes
    • Drains
    • Nebulizer use

Additional Information

  • Floating: Not required
  • Charge Nurse: Not required
  • Travel Radius: Typically limited to a 30-mile radius unless emergent needs arise
  • Transportation: Reliable personal or rental vehicle strongly recommended; public transportation is not accessible in the area
  • Vehicle Requirements: All-season or snow tires required November through April
  • Location Coverage: Chittenden County and Grand Isle County
  • Shift Length: 10-hour shifts are not accommodated

This position is ideal for experienced Home Health nurses with strong clinical skills, prior travel experience, and proficiency in multiple EMRs, seeking a travel assignment with a manageable caseload and supportive orientation.
