Licensed Practical Nurse (LPN) - Contract Position

Job Summary

The Licensed Practical Nurse (LPN) performs and documents assessments appropriate to licensure. Responsibilities include completing health history and admission processes, making interventions or referrals, identifying discharge planning and education needs, providing patient and personal care, and communicating patient progress while managing workload.

Key Responsibilities

• Assist registered nurses or other licensed practitioners in identifying patient needs, care priorities, and goals considering patient demographics and beliefs.
• Contribute to evaluating and modifying the plan of care by observing, recording, and communicating patient responses.
• Demonstrate competency in department-specific functions according to standards, policies, and procedures.
• Function as part of a multidisciplinary team to implement integrated, patient-centered care plans.
• Delegate nursing interventions to qualified personnel safely.
• Identify early warning signs of patient condition changes and report appropriately.
• Implement nursing interventions and prescribed medical regimens timely and safely, documenting care accurately.
• Collaborate and communicate relevant patient information with patients, families, and team members to ensure quality and continuity of care.
• Promptly report patient self-reported symptoms of pain according to procedures.
• Participate in health education approved by licensed practitioners.
• Assist in discharge planning, education, and counseling to optimize patient and family understanding.
