Job Opportunity: Home Health Physical Therapist (Travel/Contract)
Location: Seaside, Oregon
Position Details
- Specialty: Therapy/Rehabilitation (Physical Therapy)
- Employment Type: Travel/Contract
- Contract Length: 13 weeks
- Start Date: ASAP
- Shift: 8-hour day shifts (No 4x10 or 3x12 shifts)
- Patient Load: 1:1 ratio; up to 6 patients per day; 30 productivity points standard
- Years of Experience Required: Minimum 1 year of skilled home health experience
- First-Time Travelers: Accepted
- Weekend Work: Not required
- On-Call: Not required
- Floating: Not required
- Block Scheduling: Not accommodated
- Locals: Accepted within 50-75 mile radius, paid at same rate
- Pending License: Accepted
- Transportation: Traveler must have own transportation
- Travel Radius: Up to 60 miles per day (60-90 minutes total drive time)
- COVID-19 Vaccination: Primary series plus one booster required
- Documentation: OASIS documentation required
- Guaranteed Hours: Facility may call off up to one shift per pay period (every two weeks)

- Charting System: EPIC Remote Client (laptop) and Rover app (phone) for OASIS

Required Certifications
- BCLS (Basic Cardiac Life Support) - Required
- Other certifications (ACLS, PALS, NRP, etc.) may be required based on clinical track but not specified for this role
Licensure & Background Requirements
- Must have valid state license (pending license accepted)
- Background check within 45 days of start, covering all locations lived/worked in past 7 years
- Various state-specific background and abuse/neglect searches may be required depending on placement
- Employment application covering at least last 5 years required
- Minimum of 2 professional references covering 1 year of employment in a comparable specialty/setting (6 months for CNAs)
- Physical exam within 1 year of start, signed by MD, NP, PA, or DO, indicating ability to work and free of infectious disease
- Drug screen: 10-panel within 90 days of start (instant tests not accepted)
- Immunizations and health documents required:
- COVID vaccine or declination
- Flu vaccine during flu season or declination
- Tetanus/Diphtheria/Tdap within 10 years or declination
- Negative TB test or chest x-ray within 12 months, updated annually
- MMR (Measles, Mumps, Rubella) vaccination or positive titer or declination
- Varicella vaccination or positive titer or declination
- Hepatitis B vaccination or signed declination
Additional Requirements
- Fit mask test required for respirator use, updated annually (travel only)
- Respiratory safety clearance required annually for travel candidates
- Must complete required WorkRamp training modules prior to start (Oregon Core Modules for profession)
- Badge photo required for facility ID
- No floating between units expected, but float policy acknowledgment required
- References and employment history must be verifiable and meet specified standards
