Job Opportunity: Registered Nurse (Home Health) - Washington, Lacey

Position Details

  • Specialty: Registered Nurse (Home Health)

  • Guaranteed Hours: 40 hours per week
  • Average Patients per Day: 5-7 (to be confirmed during interview)
  • Minimum Weekly Points: 30
  • Types of Cases: Geriatric
  • Territory: Up to a 60-mile radius from the hub/office
  • Locals Accepted: Yes, at travel rate
  • Dress Code: Scrubs (any color), no jeans
  • Device Provided: iPad
  • Cell Phone: Not allowed
  • Mileage Reimbursed: Yes, at IRS standard rate

Experience & Requirements

  • Years of Experience Required: Minimum 1 year recent experience in Home Health nursing
  • OASIS Experience: Required
  • EMR Experience: HCHB (Homecare Homebase) required
  • Certifications Required: BLS (Basic Life Support)
  • License: Must have current license in hand
  • First Timers: Not accepted (prior travel experience required)
  • On-call: Yes, details to be discussed during interview
  • Special Requirements:
    • Flexibility with territory expected
    • Timely documentation and communication required

Compliance & Background Checks

  • Background checks must be current within 90 days of start and updated annually, including:
    • SSN Trace
    • County searches (last 7 years)
    • FACIS Level 1
    • Nationwide Sex Offender
    • Motor Vehicle Records/Driver’s Record
    • Education Verification
    • Work History Verification (7 years)
    • EPLS/GSA/SAM
    • HHS/OIG
    • Fingerprinting (WA-specific on-site fingerprinting required if candidate has not lived in WA for past 3 years)
    • State-specific criminal history and employability status checks as applicable

Certifications & Competencies

  • BLS (AHA, ARC, Military acceptable)
  • Orientation Safety Manual Acknowledgement (updated annually)
  • State License Verification (primary source within 90 days of start and upon expiration)
  • NURSYS Verification (for RN)
  • Medication Test/Pharmacology Competency Exam (as applicable)
  • Competency Exam (as applicable)

Health & Immunization Requirements

  • Physical Exam (within last 12 months)
  • Fit Mask Test (N95 accepted)
  • Negative TB test or Chest X-Ray (within 1 year; chest X-ray within 5 years if positive PPD)
  • TB Questionnaire (required annually)
  • Immunizations or titers for:
    • Rubeola (Measles)
    • Mumps
    • Rubella
    • Hepatitis B
    • Varicella
  • Drug Testing: 9 Panel Drug Screen within last 12 months
  • Seasonal Flu Vaccination or facility-specific declination form required
  • COVID Vaccination or facility-specific declination required

Documentation & History

  • 2 Professional References (travel, allied, per diem acute care) within 2 years of start
  • Employment Application with full 7 years of work history (no gaps)
  • Skills Checklist (annual)
  • Work History Verification (7 years, within 90 days of start)
  • Agency Background Attestation and Acceptable Use Policy must be completed prior to start
  • Proof of current auto insurance required (for positions involving driving)

This position requires a licensed RN with at least 1 year of recent home health experience, prior travel experience, and the ability to work flexible territories within a 60-mile radius. Compliance with extensive background checks, certifications, and health documentation is mandatory.
