Job Opportunity: Social Services - Home Health LMSW

Location: Portland, Oregon
Employment Type: Travel/Contract
Contract Length: 13 weeks


Job Details

  • Specialty: Social Services (Licensed Master Social Worker - LMSW)
  • Setting: Home Health (East Portland and surrounding areas)
  • Shift: 8-hour day shifts (no 4x10 or 3x12 shifts)
  • Patient Ratio: 1:1, may visit up to 6 patients per day (30 productivity points standard)
  • Start Date: ASAP
  • Weekend Requirement: No
  • On-Call: No
  • Floating: No floating between units required
  • Block Schedule: Not accommodated
  • Locals Accepted: Yes, within 50-75 mile radius
  • Pending License Accepted: Yes, but traveler must have BOTH Oregon and Washington licensure
  • Transportation: Traveler must have own transportation
  • Drive Time: Occasional max radius ~60 miles (60-90 minutes total drive time per day)
  • COVID Vaccination: Primary series plus one booster required
  • Charting System: EPIC Remote Client (laptop), Rover (phone app), and Oasis
  • Oasis Experience: Required
  • Guaranteed Hours: Facility may call off up to one shift every two weeks

Experience & Certification Requirements

  • Years of Experience: Minimum 1 year of skilled home health experience
  • Prior Travel Experience: Not explicitly required
  • Certifications Required:
    • BCLS (Basic Cardiac Life Support)
  • Licensure: Must hold valid licenses for both Oregon and Washington

Compliance & Documentation Requirements

  • Background checks including multi-state and state-specific searches (Oregon and Washington)
  • Employment application covering at least last 5 years
  • Minimum of 2 professional references covering 1 year of relevant experience within last 3 years
  • Physical exam within 1 year of start, signed by MD, NP, PA, or DO
  • Immunizations and health documents required:
    • COVID vaccine or declination
    • Flu vaccine during flu season or declination
    • Tetanus/Diphtheria/Tdap within 10 years or declination
    • Negative TB test or chest x-ray within 12 months
    • MMR (Measles, Mumps, Rubella) vaccination or positive titer or declination
    • Hepatitis B vaccination or signed declination
    • Varicella vaccination or positive titer or declination
  • Drug screening: 10-panel drug screen within 90 days of start (no instant tests)
  • Respirator fit testing required for travel candidates (updated annually)
  • Other certifications and competencies as applicable by profession

Training & Modules

  • Completion of Providence Health and Services WorkRamp modules prior to start, including:
    • Core modules for Oregon Home Health professionals
    • Specialty-specific modules as applicable
    • Annual regulatory requirements
  • EPIC training modules required within a week of orientation

Attire & Work Environment

  • Business casual attire; scrubs allowed but not required (no jeans)
  • Free parking available
  • Mileage reimbursement per IRS standard

Additional Notes

  • Facility cannot accommodate block schedules or floating between units
  • Traveler must be prepared for occasional longer drives to patient homes
  • Must comply with all Providence Health and Services policies and documentation requirements

This position is ideal for a Social Services professional with home health experience who holds dual licensure in Oregon and Washington, is comfortable with travel within the Portland area, and is ready to start immediately.
