Schedule

• Monday – Friday, 8:00 AM – 4:30 PM
• Weekend rotation: 1 out of every 4 weekends (one weekend shift per month)

Job Description

• Provide skilled nursing care in patients' homes under the direction of RN Case Manager and Clinical Team Manager
• Perform approximately 6 patient visits per day
• Work collaboratively within interdisciplinary teams including RNs, PTs, OTs, and HHAs
• Must have home care experience
• Follow State Nurse Practice Act, organizational policies, and professional standards

Required Credentials & Compliance

Licenses & Certifications

• Valid professional nursing license (Primary Source Verification required; MA Conditional Licenses not accepted)
• Basic Life Support (BLS) certification (AHA preferred)
• Driver’s License or State ID
• Resume including past 7 years of work experience (with city & state)
• Two recent references (minimum one supervisor or above)
• Skills checklist specific to the role
• Candidate submission coversheet

Health & Immunizations

• Fit Test for N95 respirator within 365 days of start date, updated annually
• Documentation of MMR immunity (vaccines or blood test)
• Influenza vaccine required during flu season (Sept 15 – April 30)
• TB test documentation within 3 months of start date (skin test or blood test)
• Health statement signed by a licensed provider within 365 days
• Hepatitis B vaccination or immunity strongly preferred
• Tdap vaccine within last 10 years preferred
• Positive PPD requires negative chest x-ray documentation (if applicable)

Background & Compliance Checks

• Criminal background check within 45 days prior to start, updated if break >6 months
• Massachusetts CORI background check within 30 days prior to start, updated if break >6 months
• Motor Vehicle Record (MVR) check within 45 days prior to start
• National Sex Offender Registry check within 30 days prior to start, updated annually
• Exclusion checks (OIG, GSA, MassHealth, NH Medicaid) within 30 days prior to start, updated annually
• Social Security Number trace as part of background check
• Education verification of highest level achieved

Additional Requirements

• MGB badge photo (color, recent, professional headshot)
• Respirator Medical Clearance attestation (part of fit test documentation)
• Confirmation of assignment form upon acceptance and for any start date changes
