Job Opportunity: Home Health Physical Therapist (Travel/Contract)

Location: Medford, Oregon
Specialty: Therapy/Rehabilitation (Physical Therapy)
Contract Length: 13 weeks


Job Details

  • Start Date: ASAP
  • Shift: 8-hour day shifts (No 4x10 or 3x12 shifts available)
  • Patient Ratio: 1:1 (May visit up to 6 patients per day; 30 productivity points standard)
  • Experience Required: Minimum 1 year of skilled home health experience
  • First-Time Travelers: Not accepted
  • Weekend Requirement: No weekend shifts required
  • Certifications Required: BCLS (Basic Cardiac Life Support)
  • On-Call: Not required
  • Floating: No floating between units required
  • Block Schedule: Not accommodated
  • Locals Accepted: Yes, within 50-75 mile radius
  • Pending License: Accepted
  • Guaranteed Hours: Facility may call off up to one shift per pay period (every two weeks)
  • Transportation: Traveler must have their own transportation
  • Travel Radius: Occasional travel up to 50+ miles; system groups clients to minimize drive time
  • COVID Vaccination: Primary series plus one booster required
  • Documentation/Charting: EPIC and Oasis required (Oasis experience preferred)
  • Attire: Any color scrubs
  • Parking: Free
  • Mileage Reimbursement: IRS standard

Required Qualifications & Experience

  • Specialty Experience: At least 1 year of skilled home health physical therapy experience (highest requirement)
  • Prior Travel Experience: Not required
  • Certifications:
    • BCLS (required)
    • Other certifications (ACLS, PALS, NRP, etc.) may be required depending on clinical track but not specified here

Compliance & Documentation Requirements

  • Background Checks:
    • Must be completed within 45 days of start, covering all locations lived/worked in past 7 years
    • Includes SSN trace, death master index, nationwide and county checks, MVR (required for home health), and others
    • Annual updates required
  • Licenses:
    • State professional license required and must be verified
    • NURSYS verification for RNs as applicable
  • Health Documents:
    • Physical exam within 1 year of start, signed by MD, NP, PA, or DO
    • Fit mask test (annual) required for travel starts after 2/5/2024
    • Immunizations/vaccinations required:
      • COVID vaccine or declination
      • Flu vaccine during flu season or declination
      • TDAP within 10 years or declination
      • MMR (Measles, Mumps, Rubella) vaccination or positive titer or declination
      • Varicella vaccination or positive titer or declination
      • Hepatitis B vaccination or signed declination
    • Negative TB test or chest x-ray within 12 months, updated annually
    • Drug screen (10 panel) within 90 days of start
    • Respiratory safety clearance annually for respirator use
  • References:
    • Minimum 2 references covering 1 year of employment within last 3 years in comparable specialty/setting
  • Employment Application:
    • Must include at least last 5 years of employment with month/year format
  • Skills Checklist:
    • Required annually as applicable

Training & Competency Requirements

  • Competency Exams: Required as applicable by profession
  • Medication/Pharmacology Competency: Required as applicable
  • WorkRamp Modules:
    • Required pre-start and annually for all caregivers except non-clinical and certain excluded roles
    • Includes core regulatory, specialty-specific, and regional modules based on facility and specialty
  • Epic Training: Required for charting; transcripts must be completed and verified prior to working

Additional Notes

  • Facility cannot accommodate alternative shift patterns such as 4x10 or 3x12
  • Traveler must be able to manage travel within a 50+ mile radius
  • Facility requires primary COVID vaccination series plus booster
  • Oasis documentation experience is preferred and should be disclosed if not possessed
  • Locals within 50-75 miles are accepted at the same rate as travelers

This position is ideal for experienced home health physical therapists who meet the certification and documentation requirements and are ready for a 13-week travel contract in the Medford, Oregon area.
