Health & Immunization Requirements
- Physical exam within 12 months of start
- Fit mask test within 1 year of start, renewed annually
- Adult Tetanus/Diphtheria/Tdap vaccine within 10 years
- Negative TB test (2-step PPD, QGOLD, or TSPOT) within 1 year
- TB questionnaire if positive TB test, renewed annually
- Measles (Rubeola), Mumps, Rubella: Positive titer or 2 vaccines required; boosters required if negative/equivocal
- Hepatitis B: Positive titer, series, or declination accepted
- Varicella: Positive titer, 2 vaccines, or physician-documented history required; boosters if negative/equivocal
- Negative 10-panel drug screen within 90 days of start
- Seasonal Flu vaccination or approved medical/religious exemption required during flu season (Oct 1 - Mar 31)
- COVID-19 vaccination or approved medical/religious exemption required (dose 1 and fully vaccinated)
Competency Requirements
- Medication Test/Pharmacology Competency Exam (80% passing or meet Angoff score)
- Color Blind Testing (online acceptable)
- Competency Exam (minimum 80% passing or meet Angoff score)
- Unit-specific Skills Checklist required annually

- Clear headshot photo required for badge
- Orientation Safety Manual Acknowledgement and HIPAA compliance required
