Therapy/Rehabilitation Travel/Contract Job - Point Pleasant, West Virginia
Setting
- Hospital Based Outpatient Therapy
- Average of 8 patients per day
- Outpatient coverage with Swing/Inpatient coverage as needed
- Facility Type: Outpatient Physical Therapy – Hospital Based
- Total Staffed Beds: 49
- Charting Systems: Meditech Expanse (Inpatient/Swing), ReDoc (Outpatient)
Contract Length
- 13 weeks
Required Certifications
- ACLS (Advanced Cardiac Life Support) – AHA only, as required
- BLS (Basic Life Support) – AHA only
- PALS (Pediatric Advanced Life Support) – AHA only, as required
- NRP (Neonatal Resuscitation Procedure) – as required
- TNCC (Trauma Nursing Core Course) – as required
- State License Verification (Primary Source Verification)
- NURSYS Verification (for RNs)
- Tech Certification Verification (for Allied Professionals)
Required Documentation & Compliance
- Government issued photo ID (Driver’s License/State ID) must be carried during hours of service
- Residency Declaration Form (if working on a compact license)
- Background Checks:
- 7-year criminal background check within 1 year of start, updated annually
- EPLS/GSA/SAM check within 30 days of start and updated for every contract renewal
- HHS/OIG check within 30 days of start and updated for every contract renewal
- Competency Exams including Medication/Pharmacology competency
- Orientation Safety Manual Acknowledgement (HIPAA compliance)
- Physical Exam: Initial and updated annually, attesting ability to meet physical demands
- Immunizations and Health Requirements:
- Tetanus/Diphtheria/Tdap vaccine within last 10 years
- Negative TB test (two-step PPD, QGold, or TSpot) within 6 months of hire; follow-up chest X-ray if positive
- Annual TB Questionnaire
- Rubeola (Measles) – titer or 2 vaccines
- Mumps – titer or 2 vaccines
- Rubella – titer or 2 vaccines
- Hepatitis B – titer, vaccine series, or declination
- Varicella – titer or 2 vaccines
- Drug Testing: 10-panel drug screen within 90 days of hire (does not require annual update)
- Seasonal Flu Vaccination/Documentation (flu season 10/1 – 3/31; medical/religious exemptions allowed with documentation)
