Job Opportunity: Therapy/Rehabilitation - Outpatient Physical Therapist

Location: Harvey, Illinois
Employment Type: Travel/Contract
Contract Length: 13 weeks


Job Details

  • Worksite: Outpatient Physical Therapy at Flossmoor location
  • Schedule: 5 days per week, 8 hours per day
  • Some evening shifts required

Experience Requirements

  • Minimum 1 year of experience in specialty or comparable/higher acuity area

Certifications & Licenses

  • Basic Life Support (BLS) - Required for all candidates (AHA provider or AHA RQI accepted; RQI-only providers not accepted)
  • Advanced Cardiac Life Support (ACLS) - Required for specific RN/Unit positions (not required for this specialty)
  • State License verification required within 30 days of assignment start
  • Illinois Department of Public Health (IDPH) Fingerprint Registry required if profession does not require an Illinois state license (must be completed in Illinois)

Background & Compliance Checks

  • 7-year background check including aliases, sex offender, national and federal criminal checks, FACIS 3, OFAC
  • EPLS/GSA/SAM and HHS/OIG checks within 30 days of start
  • Education verification required (diploma/transcripts acceptable if verification unavailable)
  • Drug testing: Negative 9-panel within 30 days of start
  • Employment verification covering last 7 years
  • Skills checklist updated annually for assigned and float units
  • References: 2 required

Health & Immunization Requirements

  • Physical exam within 1 year, updated annually
  • Fit mask test within 1 year of start (specific 3M masks required)
  • Tetanus/Diphtheria/Tdap vaccination within 10 years or declination (time sensitive)
  • Negative TB test or chest X-ray within 1 year, updated annually
  • TB questionnaire if positive TB test
  • Proof of immunity or vaccination for:
    • Rubeola (Measles)
    • Mumps
    • Rubella
    • Hepatitis B (declination allowed)
    • Varicella
  • Flu seasonal vaccination/documentation for 2025-26 season or approved medical/religious exemption (declinations not accepted)

Additional Requirements

  • Photo for badge (passport style, plain background, forward facing, no headgear)
  • Completion of facility orientation modules (~2 hours) prior to start
  • COVID-19 religious or medical exemption forms if applicable (must be submitted and approved at least 1 week prior to documentation due date)

Notes

  • Prior travel experience is not explicitly required for this position.
  • Candidates must complete fingerprinting in Illinois if their profession does not require an Illinois state license.
  • All documentation and verifications must be completed timely to avoid delays or removal from schedule.
