Job Details
- Position: Physical Therapist (PT)
- Specialty: Home Health
- Minimum Years of Experience Required: 1 year (recent experience required)
- Prior Travel Experience: Not accepted (First timers NOT accepted)
- Average Patients per Day: 5-7 (to be confirmed during interview)
- Guaranteed Hours: 30 visits weekly with a 40-hour weekly guarantee
- Types of Cases: Geriatric
- Territory: Up to a 60-mile radius from the hub/office
- Locals Accepted: Yes, at travel rate
- License: Must have Washington state or compact license in hand
- Certifications Required: BLS (Basic Life Support)
- Documentation: All charting must be completed by 9 AM the following morning
- Dress Code: Scrubs (any color), no jeans
- Technology Provided: iPad provided; cell phone use not allowed
- Mileage: Reimbursed at IRS standard rate
- Timely documentation and communication are required

- County searches for last 7 years of locations lived/worked/schooled
- FACIS Level 1
- Nationwide Sex Offender
- Motor Vehicle Records/Driver’s Record (MVR)
- Education Verification (highest degree related to profession)
- Work History Verification (7 years)
- EPLS/GSA/SAM
- HHS/OIG
- Fingerprinting (WA candidates who have not lived in WA for past 3 years must complete on-site fingerprinting)
- State-specific criminal history and abuse registry checks as applicable
Certifications
- BLS (AHA, ARC, Military accepted)
- State License Verification (Washington or Oregon license required)
- NURSYS Verification (if applicable)
- Orientation Safety Manual Acknowledgement (updated annually)
Competencies
- Medication Test/Pharmacology Competency Exam (as applicable)
- Competency Exam (as applicable)
Facility Specific Documents
- Proof of current auto insurance (required for all Home Health positions involving driving)
- Acceptable Use Policy (must be completed prior to start)
- Flu and COVID Statement (required if declining flu vaccine and/or COVID booster)
- Agency Background Attestation
Health Documents
- Physical Exam (physician statement within last 12 months)
- Fit Mask Test (N95 accepted)
- Negative TB test or Chest X-Ray (within 1 year; chest x-ray within 5 years if positive PPD)
- TB Questionnaire (required annually)
- Immunizations: Rubeola (Measles), Mumps, Rubella, Hepatitis B, Varicella (vaccines, titers, or declination accepted)
- Drug Testing (9 Panel Drug Screen within last 12 months)
- Flu Seasonal Vaccination or Declination Form
- COVID Vaccination or Declination
History & References
- 2 Professional References (travel, allied, per diem acute care) within 2 years of start
- Full 7 years of work history with no gaps required on application
- Work History Verification within 90 days of start (3 attempts required if unverifiable)
- Skills Checklist (annual)
Additional Notes
- Prior travel experience is required; first-time travelers are NOT accepted for this position.
- Timely and accurate documentation is critical.
- Candidates must comply with all background, health, and licensing requirements prior to start.
- Mileage reimbursement and technology support provided.
This position is ideal for experienced Physical Therapists with home health experience who have completed at least one prior travel contract and hold the appropriate state license.
