Job Description

• Position: Microbiologist (Medical Technologist)
• Assignment Duration: 11 weeks
• Schedule: 5 days a week, 8-hour shifts (40 hours/week)
• License: New Hampshire state license required
• Candidate must live more than 50 miles from Dover, NH

Licenses & Certifications

Health & Immunization Requirements

• N95 Fit Test within 365 days of start date (no onsite fit testing; must use specific form)
• Measles, Mumps, Rubella (MMR) vaccination or proof of immunity
• Influenza vaccine required during flu season (Sept 15 - April 30), no exemptions allowed
• Tuberculosis (TB) screening: skin test or blood test within 3 months of start date, updated if >6 month break
• Health statement signed by MD/DO/NP/PA within 365 days of start date
• Hepatitis B vaccination or immunity preferred but optional
• Tdap vaccine preferred but optional
• If positive TB test, documentation of a negative chest x-ray required

Background & Verification Checks

• Criminal background check covering past 7 years, including international addresses if applicable
• Education verification (highest level achieved)
• Social Security Number trace covering past 7 years
• National sex offender registry check within 30 days of start and updated annually
• Exclusion checks against OIG, GSA, MassHealth, and NH Medicaid lists within 30 days of start
• New Hampshire state-specific criminal and BEAS record checks within 30 days of start
• Respirator Medical Clearance attestation required if fit test is required

Documentation & Submission Requirements

• Resume including past 7 years of work experience with city and state
• Two recent professional references (minimum one supervisor or above)
• Skills checklist and candidate submission coversheet required for submission
• Confirmation of assignment/POI form required upon acceptance and for extensions

Additional Notes

• No religious exemptions for N95 fit testing or influenza vaccine
• Fit testing updates can only be done at specific locations if assignment is extended
• All documentation must be current and updated as specified to maintain compliance
