Health Requirements
- Physical Exam within 1 year of start (signed by MD, NP, PA, or DO)
- Fit Mask test (annual, travel only)
- Tetanus/Diphtheria/Tdap vaccine within 10 years (declination accepted)
- Negative TB test or Chest X-Ray within 12 months (annual update required)
- MMR (Measles, Mumps, Rubella) vaccination or positive titer
- Hepatitis B vaccination or signed declination
- Varicella vaccination or positive titer
- 10 Panel Drug Screen within 90 days of start (no instant tests)
- Seasonal Flu Vaccine or declination during flu season
- COVID vaccine or declination
- Respiratory Safety Clearance (annual for travel)
Non-clinical or remote workers may have reduced requirements.
Background and Compliance
- Background Check within 45 days of start covering all locations lived/worked in past 7 years
- EPLS/GSA/SAM and HHS/OIG checks within 30 days of start, updated monthly
- Education Verification required for all candidates
- State-specific searches (Washington WSP/WATCH, OBRA Registry for CNAs) as applicable
- References: Minimum 2 supervisor references covering 1 year employment in specialty (6 months for CNAs) within last 3 years
- Employment Application covering last 5 years (7 years for APN/Subcontractors) with month/year format
