Job Opportunity: RN - Adult Cardiac Intensive Care Unit (CVICU)

Unit Details

• 20 beds, all private and monitored
• Patient-to-nurse ratios vary by acuity: 1:1, 1:2, or 2:1
• Charge nurses and house supervisors available 24/7; charge nurses sometimes take assignments
• Patient care technicians (PCTs) present with ratios ranging from 1:12 to 1:15 or higher depending on staffing
• Respiratory therapists available 24/7
• Cardiovascular Advanced Practice Nurses (APNs) and Physician Assistants (PAs) usually on site; providers on call

Patient Population

• Post open heart surgery patients (CABG, valve repairs/replacements, dissections, AAA, vascular procedures, carotids)
• Post cardiovascular procedures including STEMI, unstable angina, unstable NSTEMI, cardiogenic shock, cardiomyopathy
• Targeted temperature management (hypothermia) patients
• Patients with transcutaneous and transvenous pacing
• Severe heart failure, TAVRs, cardiac-related ICU care
• Ventilated patients and overflow from other ICUs including COVID patients
• Soon to include post VAD placement and post heart transplant patients

Required Skills & Experience

• Minimum 2 years of solid CVICU experience required (prefer more)
• Must be able to start immediately without additional training on equipment or patient population
• Ability to work in a fast-paced, high-stress environment
• Strong assessment skills, critical thinking, communication, organization, teamwork, and compassion
• Ability to follow protocols and orders as written
• Time management and positive attitude essential
• Must be able to interpret cardiac monitor strips independently

Certifications

• BLS required
• ACLS preferred
• CCRN certification is a plus

Clinical Responsibilities

• RN required to titrate multiple drips including dopamine, dobutamine, levophed, epinephrine, norepinephrine, vasopressin, cleviprex, propofol, fentanyl, versed, insulin, heparin, esmolol, among others
• Manage specialty cardiovascular equipment such as:

  • Intra-aortic balloon pumps (IABPs)
  • Impella devices
  • Swan-Ganz catheters
  • ECMO
  • External pacemakers
  • Continuous renal replacement therapy (CRRT)
• Manage lines including PICC, midlines, central venous catheters (CVC), Swan-Ganz, arterial lines (radial and femoral)
• Care for patients with temporary pacemakers and multiple electrophysiology (EP) patients
• Ventilator management including CPAP, BiPAP, heated high flow, and nitric oxide troubleshooting

Monitoring & Equipment

• Philips telemetry monitoring system used
• RNs expected to read and interpret telemetry strips; monitor techs available

Scheduling & Work Environment

• 12-hour shifts (7am-7pm or 7pm-7am)
• Weekend requirement: every other weekend (day shift weekends are Saturday and Sunday; night shift weekends are Friday and Saturday)
• Holiday requirement: every other holiday
• No self-scheduling or block scheduling for travelers
• Floating required to all levels of care; travelers expected to take a full team
• Dress code: no specific scrub color required, but appearance must be professional and clean; scrubs not provided

Additional Notes

• Proficiency in all cardiovascular ICU components is required except immediate post-op care (which is a bonus)
• High acuity patient care with multiple complex devices and drips on one or more patients simultaneously
