Job Description

• Perform patient evaluations and develop, implement, and adjust individualized treatment plans.
• Collaborate with healthcare professionals to help patients reach maximum functional levels.
• Provide direct patient care, evaluate and reassess functional needs, and consult with specialists as needed.
• Conduct therapy within standards set by the Illinois Physical Therapy Practice Act and department procedures.
• Delegate and supervise treatment provided by Physical Therapist Assistants (PTA) and support staff.
• Document evaluations, treatment goals/plans, and maintain required prescriptions and signatures.

Qualifications

• Graduate of an accredited Physical Therapist program.
• Must possess Illinois Physical Therapist license or temporary work permit.
• Prior experience preferred but not explicitly required.
