Job Opportunity: Physical Therapist (Outpatient) - Travel/Contract
Location: Cortlandt Manor, New York

Shift:
- Monday: 10:00 AM – 8:30 PM
- Tuesday: 8:00 AM – 5:30 PM
- Wednesday: 10:00 AM – 8:30 PM
- Thursday: 8:00 AM – 5:00 PM
Job Description
- Responsible for evaluation, planning, directing, and administering treatments to restore function and prevent disability following injury.
- Outpatient setting with EPIC experience highly preferred.
Required Qualifications
- Current, active New York State Physical Therapist (PT) License.
- CPR card preferred.
- Experience in outpatient physical therapy required.
- Criminal background check required within 6 months prior to assignment.
- Must meet all facility and state compliance requirements including education verification and various background screenings.
Certifications & Competencies
- ACLS (Advanced Cardiac Life Support) – AHA Primary Source Verification required.
- BLS (Basic Life Support) – AHA Primary Source Verification required.
- PALS (Pediatric Advanced Life Support) – AHA Primary Source Verification required.
- Medication Test/Pharmacology Competency Exam as applicable.
- Color Blind Testing (online and/or in-person if required).
- Competency Exam as applicable.
Health & Immunization Requirements
- Physical exam within 12 months of start, including vital signs, review of systems, medical history, and clearance to work without restrictions.
- Fit Mask test within 1 year of start (specific N95 masks required).
- Tetanus/Diphtheria/Tdap vaccine required if working with pediatrics or children.
- Negative TB test or chest X-ray per facility protocol.
- Annual TB Questionnaire.
- Measles (Rubeola), Mumps, Rubella vaccinations or positive titers with booster requirements if low titer.
- Hepatitis B antibody/antigen titers and declination form within 6 months of start.
- Varicella vaccination or positive titer with booster requirements if low titer.
- Negative 9-panel drug screen within 60 days prior to start (excluding marijuana).
- Hepatitis C titer within 6 months of start.
- Seasonal flu vaccination required during flu season (Sept 3 – May 31) with no declinations accepted except approved medical or religious exemptions.
- Chain of custody form required with all drug screens.
- Vision screening (Snellen) within 12 months of start; corrective lenses noted if needed.
- Color blind exam if failed online test twice (in-person Ishihara test required).
Background & History Requirements
- Criminal background check covering all locations lived, worked, or attended school in the past 7 years, including aliases.
- EPLS/GSA/SAM, HHS/OIG, Social Security Verification, Sex Offender Search, OFAC Sanctions List Search required within 30 days of start.
- Education verification for degree/certificate and highest education level achieved.
- 20-year employment application and work history verification required (including relevant and non-relevant employment).
- Two professional references within 2 years of start.
- Skills checklist required annually.
Facility Specific Requirements
- Completion of facility-specific orientation and training modules prior to or at start.
- Signed confidentiality agreement, staff attestation, harassment factsheet, and code of conduct as applicable.
- New York State Central Registry (SCR) and Statewide Exclusion List (SEL) forms and results as required.
- Hepatitis B declination form required for all candidates.

- Prior travel experience is not explicitly required for this position.

- This is a full-time equivalent (FTE) leave of absence coverage assignment.
