Job Summary

The Physical Therapist provides physical therapy services to residents to maximize their functional abilities and independence in activities of daily living.

Key Responsibilities

• Perform evaluative and therapeutic procedures under physician or healthcare provider supervision.
• Evaluate residents and establish therapeutic plans of care with functional goals, frequency, duration, and modalities.
• Collaborate with residents, caregivers, physicians, and healthcare team members regarding care plans and discharge needs.
• Monitor and adjust treatment plans based on resident progress.
• Document all interactions and treatments in the Electronic Medical Record according to department and reimbursement requirements.
• Recommend and instruct on appropriate assistive devices.
• Provide education to residents and caregivers; document teaching outcomes.
• Collaborate with Physical Therapist Assistants and other therapy professionals.
• Comply with Medicare, Medicaid, and other reimbursement regulations.
• Provide in-service education and maintain clinical skills.
• Assist in developing new rehabilitation programs and services.
• Supervise assistant-level staff as appropriate, ensuring compliance with state and professional standards.
• Mentor new staff during orientation.
• Maintain cleanliness and infection control in clinical areas.
• Maintain required licenses, certifications, and complete annual compliance courses.

Qualifications

• Bachelor’s degree required; Master’s or Doctorate preferred.
• Active Physical Therapist license in Massachusetts.
• Basic Life Support (BLS) certification preferred.
• Prior experience in long-term care or skilled nursing facility preferred.
• Strong verbal, written, and presentation communication skills.
• Ability to work collaboratively in an interdisciplinary team.
• Excellent time management and documentation skills.
• Compassionate, resident-centered care approach.
• Proficient in Microsoft Office.
• Ability to understand and follow written instructions in English.

Physical Demands

• Ability to perform tasks involving sitting, standing, walking, bending, stooping, twisting, and reaching.
• Must be able to lift up to 50 pounds; lifting over 50 pounds requires two-person assistance.
• Frequent lifting/carrying up to 20 pounds.
