Required Licenses and Certifications

• Valid Registered Nurse (RN) license in Ohio (Primary Source Verification within 30 days of start; must be renewed if expiring within 30 days of start)
• BLS (Basic Life Support) - AHA or American Red Cross; RQI acceptable
• ACLS (Advanced Cardiac Life Support) - AHA or American Red Cross; RQI acceptable
• PALS (Pediatric Advanced Life Support) preferred but not required for this specialty
• Other certifications as applicable to the unit or specialty

Background and Compliance Requirements

• Government-issued photo ID (Driver’s License or State ID) required and must be carried on assignment
• Comprehensive background check including FCRA compliant 7-year search, SSN trace, sex offender registry, OFAC, FACIS Level 3, education verification
• Monthly EPLS/GSA/SAM and HHS/OIG checks
• FBI and BCI fingerprinting within 12 months of start, updated annually (code 4723.09 for nursing)
• Fingerprinting must be completed within 48 hours of contract signing; candidates may start with results pending but must complete prior to extension

Health and Immunization Requirements

• Physical exam within 1 year of start, including Snellen eye exam (required for new candidates signing 4/11/2025 or later)
• Fit mask test (3M N95 1860 or 1870+) within 12 months, updated annually
• Tetanus/Diphtheria/Tdap vaccine every 10 years (no declinations accepted)
• Negative TB test (2-step PPD, QGold, or TSPOT) within 12 months or chest X-ray if past positive
• Immunizations or positive titers for:

  • Rubeola (Measles)
  • Mumps
  • Rubella
  • Hepatitis B (titer, series, or declination)
  • Varicella
• Annual flu vaccination or approved medical/religious exemption (declinations not accepted)
• Standard 10-panel drug screen within 12 months (instant drug screens not accepted)
• Eye exam within 1 year for new candidates (Snellen exam)

Orientation and Competency

• Completion of facility-specific online training modules prior to start (must be done on a computer using Google Chrome)
• Pass/Fail testing for EKG and medication dosing during orientation (not required for legacy candidates with less than 6 months gap between contracts)
• Unit-specific competency exam required within 12 months of start and updated annually
• Medication test/pharmacology competency exam as applicable

Employment History and References

• Employment application must include 3 years prior work history with at least 2 years clinical practice in acute care in a similar area
• Two professional references from within one year of start, from a manager, supervisor, charge nurse, or director (co-worker references not accepted)
