Job Opportunity: Home Care Occupational Therapist

Contract Length: 13 weeks

Job Overview

• Position: Occupational Therapist (OT) in Home Care
• Patient Population: End-stage patients receiving home hospice care across various settings including home, assisted living facilities, skilled nursing facilities, group homes, independent living, and hospitals for general inpatient care (GIP).
• Average Patient Census: Approximately 105 patients/day
• Caseload: RN case managers typically manage 14-16 patients each (for reference)

Schedule & Work Environment

• Shift: 1st shift, 8:00 AM to approximately 4:30 PM, Monday through Friday
• Weekend Rotation: No
• On Call: No
• Holiday Work: No
• Orientation: 1 week of unit orientation
• Floating: Within the community to patient homes
• Unit Culture: Emphasizes teamwork and team support
• Scrub Color: Blue or black
• Parking: Free parking available at office locations

Required Qualifications

• Valid RN license in Wisconsin
• Minimum 2 years of Home Health experience, including experience within the last year
• Case Management experience
• Hospice experience
• Basic Life Support (BLS) certification
• Associate of Science in Nursing (ASN) or Associate Degree in Nursing (ADN)

Additional Notes

• Prior travel experience is preferred but not explicitly required
• No weekend, holiday, or on-call responsibilities
• Support available within the department is not specified
