Required Certifications

Required Licensure

• Valid Registered Nurse license for Illinois
• Primary source verification must be completed within 30 days of assignment start

Requirements/Qualifications

• Valid RN license in Illinois
• Minimum 1 year of ICU (Intensive Care Unit) experience within the last year
• BLS certification issued by the American Heart Association
• ACLS certification issued by the American Heart Association
• Associate of Science in Nursing / Associate Degree in Nursing

Required Documentation & Compliance

• Valid Driver’s License or State ID
• Background checks including 7-year history and multiple federal/state databases
• Education verification (ADN or BSN required)
• Physical exam within 1 year, updated annually
• Fit mask test within 1 year (specific mask models required)
• Immunizations and titers (must be current or have approved exemptions):

  • Tetanus/Diphtheria/Tdap
  • Negative TB test or chest X-ray (if positive TB history)
  • Rubeola (Measles)
  • Mumps
  • Rubella
  • Hepatitis B
  • Varicella
• Drug testing: Negative 9-panel within 30 days of start
• Flu vaccination or approved medical/religious exemption (seasonal requirement)
• Photo for badge (passport style, plain background)
• Skills checklist updated annually for assigned and float units
• Employment verification covering last 7 years
• References: Minimum 2 required
• Employment application including explanation of any employment gaps over 30 days

Additional Notes

• Candidates must complete facility-specific orientation modules prior to start (approximately 2 hours)
• Illinois Department of Public Health fingerprinting required if no Illinois state license
• Religious and medical exemption forms for COVID-19 and influenza vaccines must be submitted and approved at least one week prior to documentation due date
• Prior travel experience is not required for this position
