Job Opportunity: Hospice RN (Days, M-F with Call)
Location: Missoula, Montana
Employment Type: Contract
Contract Length: 13 weeks
Job Summary
The RN Case Manager provides skilled nursing, care, and case management for patients in their place of residence. Responsibilities include admitting patients, coordinating and integrating services to ensure continuity of care, facilitating quality clinical and cost outcomes, and managing resources needed by patients or families. The role involves assessment, planning, implementation of care, patient/family instruction, and evaluation through an interdisciplinary team approach.
This position may start as a Visiting RN and transition to Case Manager, with the same clinical responsibilities for both roles.
Essential Duties and Responsibilities
- Provide skilled nursing and coordinate care to patients and families in a residential setting using a team approach and systematic evaluation authorized by a physician.
- Observe, assess, evaluate, and plan interventions related to patients’ physical and psychosocial status, including changes in condition or reactions to treatments/medications.
- Oversee care provided by Home Health Aides and CNAs, ensuring compliance with the plan of care.
- Ensure continuity of care through effective communication, information exchange, and case conferences with other disciplines and community health/support agencies.
- Deliver quality care and symptom management to promote positive clinical outcomes and optimal health through teaching, counseling, interventions, and referrals.
- Maintain accurate, current, and complete patient records in accordance with agency standards and regulatory requirements.
- Participate in team scheduling coordination and on-call rotation as needed to meet patient care needs, including telephone assessments and home visits.
- Engage in quality improvement activities and attend in-service training as assigned.
Required Qualifications
- Current RN license in the State of Montana.
- Minimum of 2 years recent experience in acute care, home care, or hospice nursing.
- Clinical skills preferred include venipuncture, catheterization, wound care, and/or parenteral therapy.
- Valid driver’s license, appropriate auto insurance, and reliable transportation.
- BSN preferred but not required.
Additional Information
- Work schedule is Monday through Friday with call responsibilities.
