Description

Registered Nurse Case Manager - Contract

Location: Glenwood Springs, Colorado
Employment Type: Contract
Contract Length: 13 weeks


Job Summary

Provide case management services to inpatients and outpatients, assisting with the overall management of their plan of care across the continuum. This is a full-time day position with weekend rotation and no on-call duties.


Schedule

  • 3 shifts per week
  • 12-hour shifts
  • Weekend rotation

Required Qualifications

  • Graduate of an accredited or state board-approved Registered Nursing program
  • Current Registered Nurse (RN) license in good standing
  • Minimum of 3 years RN Case Management experience (highest required years of experience)
  • EPIC electronic health record experience is required
  • Flu vaccine required (no exemptions)

Additional Information

  • Potential housing available: furnished studio and one-bedroom apartments near the facility, subject to availability
  • Travel candidates must have a permanent address at least 75 miles from the facility
