Job Opportunity: Speech-Language Pathologist (SLP) - Acute Care

Location: Kingman, Arizona

Job Description

• Position in acute care setting.
• Flexible scheduling with consideration for time off.

Required Qualifications

• Valid Speech Language Pathologist license in Arizona

Required Documentation & Compliance

• Government-issued photo ID (Driver’s License or State ID).
• Background checks including:

  • 7-year search within 30 days of start (updated annually).
  • SS Trace, County searches for last 7 years of locations lived/worked/school.
  • Nationwide Sex Offender check.
  • Education verification (transcript/diploma or national verification).
  • EPLS/GSA/SAM and HHS/OIG checks (annual).
• Copy of Transcript or Diploma required (or national verification if unavailable).
• Certifications as applicable (e.g., ACLS, BLS, PALS, NRP) depending on role requirements.
• Competency exams as applicable by profession/specialty.
• Clear, passport-style photo for badge.
• Facility-specific forms and agreements (security, confidentiality, behavioral expectations, HIPAA, etc.).
• Physical exam within 1 year of start date.
• Negative TB test or chest X-ray within 1 year of start.
• Annual TB questionnaire.
• Immunization or titer proof for:

  • Measles (Rubeola)
  • Mumps
  • Rubella
  • Hepatitis B
  • Varicella (Chicken Pox)
• Negative 9-panel drug screen within 30 days of start.
• Seasonal flu vaccination or declination during flu season (Oct 1 - Mar 31).
• Two professional references within 3 years of start.
• Annual skills checklist.
