Job Opportunity: Occupational Therapist - Acute Care Hospital

Location: Long Prairie, Minnesota
Employment Type: Travel/Contract
Contract Length: 13 weeks
Shift: 8:00 AM – 4:30 PM
Work Setting: Approximately 60% hospital (general rehab), 40% outpatient (hands/upper extremity)
Patient Population: Adults of all ages
Travel Requirement: Traveler's primary residence must be at least 25 miles away from the facility


Job Description

  • Provide occupational therapy services in both hospital and outpatient settings.
  • Outpatient focus includes hands and upper extremity therapy.
  • Hospital focus includes general rehabilitation.

Required Qualifications

  • Valid Minnesota Occupational Therapist (OT) license.
  • Basic Life Support (BLS) certification.
  • Experience with Epic electronic health record system.

Licensure & Certification Notes

  • Temporary MN OT license available quickly; fingerprint results are NOT required for temporary license issuance.
  • Permanent MN OT license requires background check and fingerprint clearance.
  • BLS certification is mandatory.

Background & Compliance Requirements

  • Criminal background check within one year of start date, including:
    • 7-year search
    • Social Security trace
    • County and nationwide checks
    • Sex offender registry
    • Education verification (diploma acceptable in lieu of third-party verification)
    • International searches if candidate has lived/worked outside the U.S.
  • Must complete Minnesota DHS Net Study Fingerprint Questionnaire immediately after signing contract.
  • Minnesota law requires fingerprint clearance or approved exemption before starting work.
  • Physical exam within 1 year of start.
  • Immunizations and health screenings required:
    • Tdap vaccination (no religious declinations accepted)
    • Negative TB test or chest x-ray per MN state guidelines
    • MMR (Measles, Mumps, Rubella) proof of immunity or vaccination
    • Hepatitis B vaccination, positive titer, or declination
    • Varicella immunity or vaccination
    • Seasonal flu vaccination or declination (mask required if declined)
  • Drug testing (9-panel) within 1 year of start (not required for client clearance but must be completed).
  • Two professional references from recent employers within 2 years.
  • Skills checklist and work history verification required.

Additional Information

  • Education modules must be completed prior to or during orientation.
  • Dress code must be reviewed prior to start.
  • Candidates must submit a professional photo for badge issuance.
  • Candidates must return badges on last day of assignment.
  • No prior travel experience is explicitly required for this position.
