Physical Therapist - Home Health
Location: Newton, Massachusetts
Employment Type: Long-Term Assignment (LTA)
Contract Length: 13 weeks
Job Summary
Provide physical therapy services to patients in their homes who have physical impairments, functional limitations, or disabilities due to injury, disease, or other causes. Patient ages range from birth to over 100 years. Collaborate with patients, families, physicians, and other health professionals to identify needs and coordinate care. Maintain thorough and timely electronic documentation and interdisciplinary communication according to agency policies and standards of practice. Weekend rotations occur once every 6 weeks. Ability to travel to assigned cases is required.
Required Qualifications
- Valid Physical Therapist license in Massachusetts
- Minimum 1 year of physical therapy experience in a Home Health (Home Care) setting within the last year
Required Credentials and Compliance (selected highlights)
- Professional license verification (primary source) within 30 days of start; no conditional licenses accepted.
- Criminal background check, including Massachusetts CORI and national sex offender registry, within 45 days prior to start and updated as required.
- Motor Vehicle Record (MVR) check within 45 days prior to start (required for all home care positions).
- Documentation of immunizations and health screenings including:
- MMR (measles, mumps, rubella) vaccination or proof of immunity
- TB test or blood test within 3 months of start, updated if break >6 months
- Health statement signed by a licensed provider within 1 year of start
- Fit testing for N95 respirator within 1 year of start, updated annually (no exemptions allowed).
- Education verification of highest degree achieved.
- Resume including past 7 years of work experience with city and state.
- Two professional references within 4 years, including at least one supervisor or higher.
- Skills checklist completed within 1 year of submission.
- Additional background and exclusion checks as required by state regulations.
Additional Information
- Weekend rotations are infrequent (once every 6 weeks).
- Travel to patient homes is a required part of the role.
- Electronic documentation and interdisciplinary communication are essential components of the job.
Summary
This position is ideal for a licensed Physical Therapist with at least 1 year of home care experience who is comfortable traveling to patient residences and working independently. Compliance with extensive credentialing, background checks, and health requirements is mandatory.
