Job Opportunity: Physical Therapist Home Health

Location: Northampton, Massachusetts
Employment Type: Long-Term Assignment (LTA)
Contract Length: 13 weeks

Job Description

Provide physical therapy services to patients in their homes according to the plan of care. This role requires specialized knowledge, judgment, and skill in physical therapy principles. The Physical Therapist will work within established policies and procedures and collaborate with the clinical rehabilitation team.

Required Qualifications

• Valid Physical Therapist license in Massachusetts
• Bachelor’s degree in Physical Therapy from an accredited program
• Minimum of 1 year of physical therapy experience, including experience within the last year
• Experience in a Home Care setting
• Strong verbal and written communication skills and excellent interpersonal skills
• Valid driver’s license, good driving record, and reliable transportation

Additional Requirements

• Documentation of immunizations or proof of immunity for MMR (measles, mumps, rubella)
• TB test documentation within 3 months of start date, updated if break >6 months
• Physician health statement within 1 year of start date, updated if break >6 months
• Criminal background checks, including state and national registries, within required timeframes
• Skills checklist completed within 1 year of submission
• Respirator fit test (N95) within 1 year of start date, updated annually
• Motor vehicle record check within 45 days of start date (required for home care roles)
• Color badge photo taken within past 6 months
