Job Description

Join a dedicated team delivering outstanding home health and hospice services that enrich the lives of patients in the community. As a Physical Therapist, you will:

• Screen, evaluate, re-evaluate, and treat patients in their homes.
• Develop and carry out appropriate treatment plans in accordance with agency policies and professional standards.
• Coordinate and communicate treatment activities with interdisciplinary team members and patients.
• Document evaluations, treatment goals, and objectives.
• Educate patients and family members on treatments and procedures.

Required Qualifications

• Valid Physical Therapist license in Vermont
• Minimum 1 year of Physical Therapist experience, including experience within the last year
• Current Basic Life Support (BLS) certification
• American Speech-Language-Hearing Association (ASHA) certification
• Certified Hand Therapist (CHT) credential
