Job Description

• Provide and coordinate home nursing care according to physician orders and agency policies.
• Directly supervise and evaluate Home Health Aides and other Licensed Practical/Vocational Nurses.
• Perform home visits with an average patient load of 5-8 patients per day.
• Types of cases include Start of Care, Wound Care, Follow-up Visits, etc.
• Must have reliable transportation and a current driver's license.
• Must be willing to travel up to 75 miles from the home clinic, averaging 150-200 miles per week.
• Charting is done using HomeCare HomeBase (HCHB); proficiency required within the last 6 months or retraining required.
• Typical schedule is Monday through Friday, 8 AM to 5 PM, with no weekend rotation or on-call required.
• Dress code: Scrubs.

Required Qualifications

• Valid LPN / LVN license in Indiana
• Minimum 1 year of recent Home Health experience within the last year, including experience working with adult patients
• Hospice experience
• BLS certification issued by the American Heart Association or American Red Cross
• Experience working in a Home Health setting
• Proficiency with HomeCare HomeBase (HCHB) EMR system

• Weekend and on-call requirements vary by location and should be discussed during the interview.
• Local candidates are accepted under the same terms as travelers.
• No guaranteed hours; if patient census is low, floating to a nearby clinic may be arranged.
• Must be willing to comply with all required background checks, drug screening, and health documentation including:

  • Physical exam within 12 months of start.
  • Negative TB test or chest X-ray within 12 months.
  • Immunizations or titers for Measles, Mumps, Rubella, Hepatitis B, Varicella.
  • 10-panel drug screen.
  • Seasonal flu vaccination or declination.
  • COVID vaccination or declination.
• Must complete all required compliance, confidentiality, and policy training prior to start.

Travel Experience

• Prior travel experience is not explicitly required, but recent specialty experience is mandatory.
